Pramipexole diHCl monohydrate
Indications/Uses
As monotherapy or in combination w/ levodopa for idiopathic Parkinson's disease. Symptomatic treatment of idiopathic restless legs syndrome.
Dosage/Direction for Use
Parkinson's disease 0.125 mg Initially 0.125 mg tid. Slowly titrate, not more frequently than every 5-7 days for 7 wk to achieve optimal response. 0.25 & 1 mg Initially 0.375 mg daily in 3 divided doses, then increase gradually to 0.125 mg/dose every 5-7 days. Range: 1.5-4.5 mg daily. Restless legs syndrome 0.125 mg once daily 2-3 hr before bedtime. Increase every 4-7 days. Max: 0.125 mg 0.5 mg/day. 0.25 & 1 mg 0.75 mg/day. 
Administration
May be taken with or without food: May be taken w/ meals to reduce GI side effects.
Contraindications
Hypersensitivity.
Special Precautions
Renal impairment. Hallucinations & confusion. Orthostatic hypotension may occur; risk of hypotension or poorly tolerated transient hypotensive episodes. May impair ability to drive or operate machinery. 0.125 mg: Melanoma, rhabdomyolysis, somnolence. New or worsening dyskinesia. Restless leg syndrome rebound & augmentation. Impulse control & compulsive behaviors or hallucinations. Fibrotic complications (peritoneal fibrosis, pleural fibrosis & pulmonary fibrosis)
Adverse Reactions
Postural hypotension, dizziness, headache, somnolence, insomnia, hallucinations, abnormal dreams, nausea, constipation, weakness, dyskinesia, extrapyramidal syndrome. 0.125 mg: Orthostatic hypotension, amnesia, asthenia, confusion, dizziness, dream disorder.
Drug Interactions
Cimetidine, CNS depressants, dopamine antagonists. Contraindicated w/ sulpiride & amisulpride. May impair antiparkinson effect w/ 1st & 2nd antipsychotics, amisulpride, sulpiride & metoclopramide. May increase risk of adverse effects w/ cimetidine, MAOIs, methylphenidate & alcohol.
