In hyperlipidemia: Adjunct to diet to reduce elevated total-C & LDL-C level in primary hypercholesterolemia & mixed dyslipidemia (IIa & IIb). In CHD: Reduce risk of death, coronary death, non-fatal MI; reduce risk of undergoing myocardial revascularization procedures; reduce risk of stroke & transient ischemic attacks (TIA).
Dosage/Direction for Use
Initially 10-20 mg/day. Recommended dose: 5-80 mg/day. To be taken as a single dose in the evening. Adjust dose at not <4 wkly interval.
Administration
May be taken with or without food: Avoid excessive consumption (>1 L/day) of grapefruit juice.
Contraindications
Active liver disease or unexplained persistent elevation of serum transaminase. Pregnancy, lactation.
Special Precautions
Myopathy. Homozygous familial hypercholesterolemia. LFTs should be performed before treatment begins & periodically thereafter (eg, semi-annually) for the 1st yr of treatment & after dose elevation in all patients. Sleep disturbance; photosensitivity.
Potent CYP3A4 inhibitors, gemfibrozil & other fibrates & niacin may increase risk of myopathy. Propranolol decrease antihyperlipidemic activity. May increase anticoagulant effect of warfarin.
