Indications/Uses
Furetic: Cardiac edema, ascites, renal, peripheral, pulmonary & cerebral edema, eclampsia & barbiturate poisoning. Furetic-S: Listed in Dosage.
Dosage/Direction for Use
Furetic Edema Adult 20-80 mg single dose. May be increased by 20 or 40 mg, no sooner than 6-8 hr after previous dose & titrated up to 600 mg/day in patients w/ severe edema. HTN 40 mg bid, adjusted according to response. Childn Initially 2 mg/kg single dose & titrated up to 1-2 mg/kg 6-8 hrly. Max dose: 6 mg/kg. Furetic/Furetic-S Edema Adult Initially 20-40 mg IM or slow IV (1-2 min). May be increased by 20 mg at least 2 hr after previous dose. Hypertensive crisis 40-80 mg IV over 1-2 min (administered w/ other antihypertensive agent). Pulmonary edema Initially 40 mg IV (over 1-2 min). If no response w/in 1 hr, may be increased to 80 mg IV (over 1-2 min). Childn Initially 1-2 mg/kg slowly IM or IV, may be increased by 1 mg/kg at intervals of 6-12 hr.
Administration
May be taken with or without food: May be taken w/ meals to reduce GI discomfort.
Contraindications
Hypersensitivity, anuria.
Special Precautions
Hepatic or renal insufficiency, prostatic hypertrophy, impaired micturition, impaired hearing, gout. Monitor fluids & electrolytes. Pregnancy & lactation.
Adverse Reactions
Abdominal discomfort, orthostatic hypotension, GI disturbances eg, nausea, diarrhea & vomiting, blurred vision, dizziness, headache, skin rashes. Fluid & electrolyte imbalance.
Drug Interactions
May potentiate antihypertensive action of tubocurarine. May enhance nephrotoxicity & ototoxicity of aminoglycosides, cephalosporins, salicylates, lithium & cardiac glycosides. Diuretic effectiveness may be reduced by probenecid, phenytoin or indomethacin. Increased orthostatic hypotension may occur w/ alcohol, narcotics, barbiturates.
