Indications/Uses
Adjunct to diet & exercise to lower blood glucose in management of type 2 DM (NIDDM). In combination w/ metformin or insulin in patients whose hyperglycemia cannot be controlled by diet & exercise in conjunction w/ single oral hypoglycemic agent.
Dosage/Direction for Use
Initially 1-2 mg once daily. Maintenance: 1-4 mg once daily, may be increased in 2 mg increments at 1-2 wk intervals after a dose of 2 mg once daily. Max dose: 8 mg once daily. Combination w/ insulin: Initially 8 mg once daily. Change over from other oral antidiabetics to glimepiride Initially 1 mg daily. Renal insufficiency Initially 1 mg daily, may titrate dose based on fasting blood glucose levels.
Administration
Should be taken with food: Take immediately before or during breakfast, or the 1st main meal of the day. Do not skip meals.
Contraindications
Hypersensitivity to glimepiride or sulfonamides. Diabetic ketoacidosis.
Special Precautions
Avoid use in patients w/ allergy to sulfonamides, sulfonylureas, carbonic anhydrase inhibitors, thiazides & loop diuretics (except ethacrynic acid). Severe hypoglycemia especially in elderly, malnourished patients & patients w/ renal & hepatic impairment. Dizziness; impaired alertness & reactions. Hemolytic anemia in patients w/ G6PD deficiency w/ sulfonylureas. Regularly check glucose levels in blood & urine; proportion of HbA1c. Galactose intolerance. Not to be used during pregnancy. Not recommended in lactation. Childn. Geriatric.
Adverse Reactions
Dizziness, headache, hypoglycemia, nausea & weakness.
Drug Interactions
Potentiated blood sugar-lowering effect w/ insulin, oral antidiabetics, ACE inhibitors, allopurinol, anabolic steroids & male sex hormones, chloramphenicol, coumarin derivatives, cyclophosphamide, disopyramide, fenfluramine, fenyramidol, fibrates, fluoxetine, guanethidine, ifosfamide, MAOIs, miconazole, p-aminosalicylic acid, quinolones, salicylates, sulfinpyrazone, sulfonamides, tetracyclines, tritoqualine, trofosfamide. Weakened blood sugar-lowering effect w/ acetazolamide, barbiturates, corticosteroids, diazoxide, diuretics, epinephrine & other sympathomimetic agents, glucagons, laxatives (after protracted use), nicotinic acid (high dose), estrogens & progestogens, phenothiazines, phenytoin, rifampicin, thyroid hormones.
