Cefuroxime axetil
Indications/Uses
URTI (ENT infections eg, otitis media, sinusitis, tonsillitis, pharyngitis). Lower resp tract infections (eg, pneumonia & acute exacerbations of chronic bronchitis). GUT (eg, pyelonephritis, cystitis & urethritis). Skin & soft tissue infections (eg, furunculosis, pyoderma & impetigo). Gonorrhoea, acute uncomplicated gonococcal urethritis & cervicitis.
Dosage/Direction for Use
Usual course of therapy: 7 days (5-10 days). Adult Most infections, UTI, mild to moderate lower resp tract infections, pyelonephritis 250 mg bd. More severe lower resp tract infections or if pneumonia is suspected 500 mg bd. Uncomplicated gonorrhoea 1 g as single dose. Childn FC tab Most infections 125 mg bd. Otitis media or more severe infections 250 mg bd. Oral susp Acute tonsillitis & pharyngitis 10 mg/kg bd. Max: 500 mg daily. Acute otitis media, acute bacterial sinusitis, community acquired pneumonia, UTI, skin & soft tissue infections 15 mg/kg bd. Max: 1,000 mg daily. Renal impairment: CrCl 10-29 mL/min Standard individual dose every 24 hr, <10 mL/min Standard individual dose every 48 hr, during hemodialysis Single additional standard individual dose at the end of each dialysis.
Administration
Should be taken with food: Take immediately after food. Do not crush.
Contraindications
Hypersensitivity to cephalosporins.
Special Precautions
Allergic reaction to penicillins or other β-lactams. May result in overgrowth of Candida; prolonged use may also result in overgrowth of other non-susceptible organisms (eg, enterococci & Clostridium difficile) which may require treatment interruption. Possible pseudomembranous colitis. Immediately discontinue treatment if prolonged or significant diarrhoea occurs or patient experiences abdominal cramps. Dizziness may affect ability to drive or operate machinery. Renal impairment. Pregnancy & lactation. Childn <3 mth. Oral susp: Advise diabetic patients about the sucrose content. Contains aspartame; patients w/ phenylketonuria.
Adverse Reactions
Overgrowth of Candida; eosinophilia; headache, dizziness; GI disturbances including diarrhoea, nausea, abdominal pain; transient increase of hepatic enzyme levels (ALT, AST, LDH).
Drug Interactions
Bioavailability may be lowered by gastric acidity-reducing drugs. Lower oestrogen reabsorption & reduced efficacy of combined OCs. False -ve result may occur in ferricyanide test.
