Indications/Uses
Symptomatic treatment of dry mouth from salivary gland hypofunction caused by radiotherapy for head & neck cancer; in patients w/ Sjogren's syndrome.
Dosage/Direction for Use
Head & neck cancer patient Initially 5 mg tds for at least 12 wk, titrate accordingly. Usual dose range: 15-30 mg daily. Max: 10 mg/dose. Sjogren's syndrome patient 5 mg qds for 6 wk.
Administration
May be taken with or without food.
Contraindications
Hypersensitivity. Patients w/ uncontrolled asthma; when miosis is undesirable [eg, acute iritis & narrow-angle (angle closure) glaucoma].
Special Precautions
Patients w/ significant CV disease. High ocular doses for acute angle-closure glaucoma may cause pulmonary edema. May increase airway resistance, bronchial smooth muscle tone & secretions. Patients w/ controlled asthma, chronic bronchitis or COPD requiring pharmacotherapy. Toxicity is characterized by exaggeration of parasympathomimetic effects. Dose-related CV pharmacologic effects including hypotension, HTN, bradycardia & tachycardia. Patients w/ known or suspected cholelithiasis or biliary tract disease. May increase ureteral smooth muscle tone & may precipitate renal colic particularly in patients w/ nephrolithiasis. Risk of dose-related CNS effects. Patients w/ underlying cognitive or psychiatric disturbances. Patient who sweats excessively & cannot drink enough liqd; dehydration may develop. May cause visual blurring, decreased visual acuity especially at night & in patients w/ central lens changes; depth perception impairment; may impair ability to drive or perform hazardous activities in reduced lighting. Pregnancy & lactation. Ped patients.
Adverse Reactions
Sweating, nausea, rhinitis, diarrhea, chills, flushing, urinary frequency, asthenia. Headache, dyspepsia, abdominal pain, vomiting, pharyngitis. Head & neck cancer patients: Dizziness. Lacrimation, edema, amblyopia, HTN. Sjogren's syndrome patients: Increased salivation. Flu syndrome, dizziness, pain, sinusitis, rash, infection.
Drug Interactions
Possibility of conduction disturbances w/ β-adrenergic antagonists. Additive pharmacologic effects w/ parasympathomimetic drugs. May antagonize anticholinergic effects of atropine, inhaled ipratropium.
