Vit D
Indications/Uses
Prevention & treatment of vit D deficiency. Adjunctive therapy to osteoporosis in patients w/ vit D deficiency or at risk of vit D insufficiency.
Dosage/Direction for Use
Adult Prevention of vit D deficiency 25,000 IU (1 amp)/mth, higher doses may be required. Adjunct to specific therapy for osteoporosis 25,000 IU (1 amp)/mth. Treatment of vitamin D deficiency (<25 ng/mL) 50,000 IU (2 amp)/wk for 6-8 wk, followed by maintenance therapy (1,400-2,000 IU/day may be required). Prevention of deficiency Paed 1-18 yr 25,000 IU (1 amp) every 6 wk, 0-1 yr 25,000 IU (1 amp) every 8 wk. Treatment of deficiency Paed 0-18 yr 25,000 IU (1 amp) once every 2 wk for 6 wk (followed by maintenance therapy of 400-1,000 IU/day).
Administration
Should be taken with food.
Contraindications
Hypersensitivity. Hypercalcaemia &/or hypercalciuria; nephrolithiasis &/or nephrocalcinosis; hypervitaminosis D; pseudohypoparathyroidism. Serious renal impairment.
Special Precautions
Monitor Ca & phosphate levels of patients w/ renal impairment. Patients receiving treatment for CVD. Monitor serum & urinary Ca levels in patients w/ sarcoidosis. Allowances should be made for the total dose of vit D. Risk of renal stones w/ concomitant Ca supplementation. Increased risk of fractures in elderly (highest increase is during the 1st 3 mth after dosing). Pregnancy (max: 2,000 IU/day) & lactation.
Adverse Reactions
Confusional state, aggression, insomnia; nausea, abdominal pain, diarrhoea, vomiting, GI disorder, dyspepsia; dizziness, flushing; hypercalcaemia, hypercalciuria, decreased appetite, tetany; pruritus, urticaria, rash, hyperhidrosis.
Drug Interactions
Reduced effect of vit D3 w/ anticonvulsants (eg, phenytoin) or barbiturates. Decreased urinary elimination of Ca w/ thiazide diuretics. Decreased effect of vit D w/ concomitant glucocorticoids. May increase risk of arrhythmia w/ drugs containing digitalis & other cardiac glycosides. Reduced GI absorption of vit D w/ ion exchange resin (eg, cholestyramine, colestipol hydrochloride, orlistat) or laxative (eg, paraffin oil). Actinomycin & imidazole antifungal agents may interfere w/ vit D activity.
