Indications/Uses
Nosocomial pneumonia; community-acquired pneumonia (CAP); complicated skin & skin structure infections including diabetic foot infections w/o concomitant osteomyelitis; uncomplicated skin & skin structure infections; vancomycin-resistant Enterococcus faecium infections including cases w/ concurrent bacteremia.
Dosage/Direction for Use
Administer IV infusion over 30-120 min. Nosocomial pneumonia, CAP including concurrent bacteremia & complicated skin & skin structure infection Duration: 10-14 days. Adult & adolescent (≥12 yr) 600 mg IV or oral every 12 hr. Childn (birth through 11 yr) 10 mg/kg IV or oral every 8 hr. Pre-term neonate <7 days 10 mg/kg IV or oral every 12 hr. May consider 10 mg/kg every 8 hr in sub-optimal clinical response. Vancomycin-resistant Enterococcus faecium infection including concurrent bacteremia Duration: 14-28 days. Adult & adolescent (≥12 yr) 600 mg IV or oral every 12 hr. Childn (birth through 11 yr) 10 mg/kg IV or oral every 8 hr. Pre-term neonate <7 days 10 mg/kg IV or oral every 12 hr. May consider 10 mg/kg every 8 hr in sub-optimal clinical response. Uncomplicated skin & skin structure infection Duration: 10-14 days. Adult 400 mg oral every 12 hr. Adolescent 600 mg oral every 12 hr. Childn 5-11 yr 10 mg/kg oral every 12 hr, <5 yr 10 mg/kg oral every 8 hr. Pre-term neonate <7 days 10 mg/kg oral every 12 hr. May consider 10 mg/kg every 8 hr in sub-optimal clinical response.
Administration
May be taken with or without food.
Contraindications
Hypersensitivity. Not to be used in patients taking products inhibiting monoamine oxidases A or B (eg, phenelzine, isocarboxazid) or taking any such products w/in 2 wk.
Special Precautions
Not indicated for gram -ve infections. Treatment >28 days. Discontinue if signs & symptoms of hyponatremia &/or SIADH occur. Myelosuppression (including anemia, leukopenia, pancytopenia & thrombocytopenia); peripheral & optic neuropathy; serotonin syndrome; mortality imbalance in an investigational study in patients w/ catheter-related blood stream infections, including those w/ catheter-site infections; Clostridium difficile associated diarrhea; patients w/ uncontrolled HTN, pheochromocytoma, thyrotoxicosis &/or patients taking directly & indirectly acting sympathomimetic agents (eg, pseudoephedrine), vasopressive agents (eg, epinephrine & norepinephrine), dopaminergic agents (eg, dopamine, dobutamine); lactic acidosis; convulsions; hypoglycemia; patients w/ phenylketonuria (PKU); development of drug-resistant bacteria. Monitor breastfed infant for diarrhea & vomiting. May reversibly impair fertility in male patients. Pregnancy & lactation. Childn. Elderly.
Adverse Reactions
Headache, diarrhea, nausea, vomiting.
Drug Interactions
Potential interaction w/ adrenergic & serotonergic agents. Increased AUC0-8 w/ vit C & E. Decreased Cmax & AUC0-12 w/ rifampin. Decreased exposure w/ other strong CYP3A4 inducers eg, carbamazepine, phenytoin, phenobarb. Reversible enhancement of pressor response to indirect-acting sympathomimetic agents, vasopressor or dopaminergic agents. Significant pressor response w/ tyramine. Reversible enhancement of pressor response of pseudoephedrine HCl or phenylpropanolamine HCl.
