Butorphanol tartrate
Indications/Uses
Management of pain; preop or preanesth medication; as a supplement to balance anesth; relief of pain during labor.
Dosage/Direction for Use
Pain IV 1 mg every 3-4 hr as necessary. Effective dosage range: 0.5-2 mg repeated every 3-4 hr depending on the severity of pain. IM 2 mg in patients who will be able to remain recumbent, in the event drowsiness or dizziness occurs, repeated every 3-4 hr as necessary. Effective dosage range: 1-4 mg repeated every 3-4 hr depending on the severity of pain. Max of single dose >4 mg. Preop, preanesth medication 2 mg IM, administered 60-90 min before surgery. Balanced anesth 2 mg IV shortly before induction &/or 0.5-1 mg IV in increments during anesth. The increment may be higher, up to 0.06 mg/kg (4 mg/70 kg), depending on previous sedative, analgesic & hypnotic drugs administered. Labor & delivery Initially 1 or 2 mg IM or IV in mothers w/ fetuses of 37 wk gestation or beyond & w/o signs of fetal distress. Do not repeat dose in less than 4 hr nor administered less than 4 hr prior to the anticipated delivery.
Contraindications
Hypersensitivity. patients dependent on narcotics; patients taking opioid analgesics chronically.
Special Precautions
Concomitant use of opioids w/ benzodiazepines or CNS depressants including alcohol. Patients who have recently received repeated doses of narcotic analgesic medication; head injury; resp depression occur especially in patients receiving other CNS active agents or patients suffering from CNS diseases or resp impairment; acute MI, ventricular dysfunction or coronary insufficiency. Discontinue in case of severe HTN. Do not consume alcohol. Patients w/ history of drug abuse or patients receiving the drug on a continuous basis for an extended period. Drowsiness & dizziness may occur. May impair the mental & physical abilities required for the performance of potentially dangerous tasks eg, driving a car or operating machinery. Pregnancy & lactation. Elderly >65 yr. Childn <18 yr.
Adverse Reactions
Somnolence, dizziness, nausea &/or vomiting.
Drug Interactions
Increased CNS depressant effects drugs that affect the CNS (eg, alcohol, barbiturates, tranquilizers & antihistamines). Increased risk of hypotension, resp depression, profound sedation, coma & death w/ other CNS depressants eg, alcohol, other sedatives/hypnotics, anxiolytics, tranquilizers, muscle relaxants, general anesth, antipsychotics & other opioids. Caution in concomitant w/ medications that affect hepatic metabolism of drugs (eg, cimetidine, erythromycin, theophylline).
