Lidocaine HCl
Dosage/Direction for Use
Inj 25-30 mg/min injected slowly in incremental doses under continual monitoring of the patient. Spray Adult Nasal procedures eg, puncture of the maxillary sinus 20-60 mg. Max: 500 mg (short procedures), 600 mg (prolonged procedures). Oral & dental procedures eg, prior to inj & procedures in the oropharynx eg, GI endoscopy 20-200 mg. Max: 500 mg (short procedures), 600 mg (prolonged procedures). Procedures in the resp tract eg, insertion of instruments & tubes 50-400 mg. Max: 400 mg (short procedures), 600 mg (prolonged procedures). Procedures in the larynx, trachea & bronchi 50-200 mg. Max: 200 mg (short procedures), 400 mg (prolonged procedures). Procedures in O & G eg, vag delivery, suturing of ruptures in the mucosa & cervical biopsies 50-200 mg. Max: 400 mg (short procedures), 600 mg (prolonged procedures). Childn Dose should not exceed 3 mg/kg for laryngotracheal use & 4-5 mg/kg for nasal, oral & oropharyngeal use.
Contraindications
Hypersensitivity to local anesth of the amide type.
Special Precautions
Bradycardia, hypotension, advanced liver disease or severe renal dysfunction, impaired CV function, partial/complete heart block. Caution use when injected into the head & neck area including retrobulbar, dental & stellate ganglion blocks. Paracervical blocks. May impair ability to drive or operate machinery. Elderly & debilitated patients. Spray: Patients w/ traumatized mucosa &/or sepsis. Avoid contact w/ eyes.
Adverse Reactions
Numbness of the tongue, lightheadedness, dizziness, blurred vision & tremors. Severe hypotension, bradycardia. Myocardium depression, decreased cardiac output, ventricular arrhythmia. Acidosis, apnea, hyperacusis. Hypoxia, hypercarbia. Spray: Sore throat, hoarseness, loss of voice.
Drug Interactions
May potentiate cardiac effects of antiarrhythmics eg, tocainide, amiodarone. Cimetidine, β-blockers.
