Levofloxacin hemihydrate
Indications/Uses
Treatment of a wide range of infections including lower resp tract infections, skin infections, typhoid & paratyphoid fever & UTI. Prophylaxis of meningococcal meningitis & for surgical infections.
Dosage/Direction for Use
Adult 250-500 mg once daily or bid for 7-14 days.
Administration
May be taken with or without food: Ensure adequate fluid intake.
Contraindications
Pregnancy & lactation. Childn, adolescents.
Special Precautions
Patient w/ epilepsy or history of CNS disorders. Adequate fluid intake should be maintained during treatment to avoid excessive alkalinity of urine because of the risk of crystalluria. G6PD deficiency or myasthenia gravis. Avoid exposure to strong sunlight. Discontinue if patients experience tendon pain, inflammation or rupture. Avoid in MRSA infection because of high level resistance. Should not be used in patients w/ history of blockages or aneurysm (abnormal bulges) vessels (ie, Marfan syndrome or Ehlers-Danlos syndrome), elderly. Stop fluoroquinolone treatment immediately if patient reports side effects suggestive of aortic aneurysm or dissection. Impaired hepatic & renal function. May impair ability to drive or operate machinery.
Adverse Reactions
Nausea, vomiting, diarrhea, abdominal pain & dyspepsia; headache, dizziness & restlessness. Rash & pruritus. Visual disturbance associated w/ bilateral uveitis.
Drug Interactions
May interfere GI absorption w/ antacids containing Ca, Mg, or Al, sucralfate, didanosine, metal cations eg, Fe, multivit prep w/ Zn. Additive effect on QT interval prolongation w/ class IA (eg, quinidine, procainamide) or class III (eg, amiodarone, sotalol) antiarrhythmic agents. Disturbances of blood glucose w/ antidiabetic agents. Increased AUC of immunosuppressive agents eg, ciclosporin & tacrolimus. Increased risk of tendon rupture w/ corticosteroids particularly in the elderly. Prolonged QT interval w/ fluconazole. Increase risk of CNS stimulation & convulsive seizures w/ NSAIDs. Prolonged elimination t1/2, elevated theophylline levels, & subsequent increase in risk of theophylline-related adverse reactions. Enhanced effects of warfarin.
