Sucroferric oxyhydroxide (equiv to Fe 500 mg)
Indications/Uses
Control of serum P levels in adult CKD patients on haemodialysis or peritoneal dialysis.
Dosage/Direction for Use
Initially Fe 1,500 mg (3 tab) daily, divided across meals of the day. Up or down titrate dose in increments of Fe 500 mg (1 tab) daily every 2-4 wk until an acceptable serum P level is reached. Max tolerated daily dose: Fe 3,000 mg (6 tab)/day.
Administration
Should be taken with food: Chew/crush tab, do not swallow whole.
Contraindications
Hypersensitivity. Hemochromatosis & any other Fe accumulation disorders.
Special Precautions
Patients w/ recent history of peritonitis (w/in last 3 mth), significant gastric or hepatic disorders & major GI surgery; diabetes (1 tab is equiv to approx carbohydrates 1.4 g or 0.116 bread u). Discoloured (black) stool. Contains sucrose & starches (carbohydrates). May be harmful to the teeth. Not to be taken by patients w/ fructose intolerance, glucose-galactose malabsorption or sucrase-isomaltase insufficiency. Earlier stages of renal impairment. Severe hepatic impairment. Pregnancy & lactation. Childn <18 yr.
Adverse Reactions
Diarrhoea, discoloured faeces. Nausea, constipation, vomiting, dyspepsia, abdominal pain, flatulence, tooth discolouration; abnormal product taste.
Drug Interactions
Monitor clinical effect & adverse events on initiation or dose-adjustment of either Velphoro or concomitant medicinal product w/ narrow therapeutic window. Administer any medicinal product known to interact w/ Fe (eg, alendronate & doxycycline) or w/ potential to interact w/ Velphoro (eg, levothyroxine) at least 1 hr prior to or 2 hr after Velphoro.
