Cilostazol
Indications/Uses
Symptomatic treatment of intermittent claudication in patients w/o rest pain & no peripheral tissue necrosis.
Dosage/Direction for Use
Intermittent claudication 100 mg bid for up to 12 wk.
Administration
Should be taken on an empty stomach: Take at least 30 min before or 2 hr after meals.
Contraindications
Hypersensitivity. CHF of any severity. Hemostatic disorders or active pathologic bleeding eg, peptic ulcer or intracranial bleeding; history of ventricular arrhythmias, QT interval prolongation, severe renal impairment or moderate to severe hepatic impairment.
Special Precautions
CV risks & lesions including endocardial hemorrhage, hemosiderin deposition & fibrosis in left ventricle, hemorrhage & necrosis of smooth muscle in coronary artery wall, intimal thickening of coronary artery & coronary arteritis & periarteritis. Severe renal (CrCl <25 mL/min) & moderate to severe hepatic impairment. Pregnancy & lactation. Childn.
Adverse Reactions
Headache; palpitations, tachycardia; dizziness, abnormal stool, diarrhea, dyspepsia, flatulence, nausea; infection & peripheral edema, asthenia, gastritis, MI, CHF, postural hypotension, insomnia, anxiety, abnormal dreams, dyspnoea, pneumonia, cough, hypersensitivity reactions, DM, anaemia, haemorrhage, thrombocythaemia, myalgia & renal impairment.
Drug Interactions
Enhances effects of anticoagulant & antiplatelet agents. Decreased metabolism w/ antifungal drugs eg, imidazole. Increased levels & effects w/ CYP2C19 inhibitors (eg, delavirdine, fluvoxamine, gemfibrozil, INH, omeprazole & ticlopidine) & CYP3A4 inhibitors (eg, azole antifungal, clarithromycin, diclofenac, doxycycline, erythromycin, imatinib, INH, nefazodone, nicardipine, propofol, PIs, quinidine, telithromycin, verapamil & grapefruit juice). Decreased levels or effects w/ CYP3A4 inducers (eg, aminoglutethimide, carbamazepine, nafcillin, nevirapine, phenobarb, phenytoin & rifamycin). Enhanced antithrombotic effects w/ dasatinib. Decreased metabolism w/ macrolide antibiotics eg, clarithromycin, erythromycin & telithromycin. Increased risk of bleeding w/ NSAIDs & salicylates. Enhanced adverse or toxic effects w/ omeprazole & trepostinil. Enhanced effects w/ omega-3-acid ethyl esters.
