Paclitaxel
Indications/Uses
Metastatic carcinoma of the ovary or breast, after failure of standard therapy.
Dosage/Direction for Use
175 mg/m2 IV over 3 hr every 3 wk. Premed prior to Unitaxel: Oral dexamethasone 20 mg 12 & 6 hr before Unitaxel, diphenhydramine 50 mg IV 30-60 min & cimetidine 300 mg prior to Unitaxel or ranitidine 50 mg IV 30-60 min before Unitaxel.
Contraindications
Hypersensitivity to paclitaxel or other drugs formulated in cremophor + EL (polyethoxylated castor oil); patients w/ severe baseline neutropenia (<1,500 cells/mm3). Pregnancy & lactation.
Special Precautions
Monitor occurrence of myelotoxicity. Perform frequent peripheral blood cell count. Monitor vital signs during 1st hr of infusion. Monitor cardiac function in patients who develop serious conduction abnormalities.
Adverse Reactions
Bone marrow suppression, neutropenia, thrombocytopenia, anemia; flushing, rash; hypotension, bradycardia; paresthesia, arthralgia, myalgia, alopecia; nausea, vomiting, diarrhea & mucositis, alopecia; bilirubin, alkaline, severe elevations in AST, alkaline phosphatase, or bilirubin.
Drug Interactions
Cisplatin enhances myelosuppressive action.
