Cilostazol
Indications/Uses
Treatment of ischemic symptoms eg, ulcer, pain & coldness of extremities in chronic arterial occlusion. Prevention of recurrence of cerebral infarction except cardiogenic cerebral embolism.
Dosage/Direction for Use
Administration
Should be taken on an empty stomach: Take at least 30 min before or 2 hr after meals.
Contraindications
Hypersensitivity; CHF. Hemostatic disorders or active pathological bleeding eg, hemophilia, increased capillary fragility, bleeding peptic ulcer, intracranial bleeding, hemorrhage in the urinary tract & vitreous body, hemoptysis, hemorrhagic stroke, proliferative diabetic retinopathy, poorly controlled HTN. Ventricular tachycardia, ventricular fibrillation or multifocal ventricular ectopics. Prolonged QTc interval. Severe tachyarrhythmia. Unstable angina pectoris, MI/coronary intervention. Concomitant (≥2) additional antiplatelet or anticoagulant agents. Infants & childn. Women of childbearing potential. Pregnancy & lactation.
Special Precautions
CV effects. Hematologic adverse reactions. Moderate or severe hepatic impairment. Severe renal impairment. Increased bleeding risk during surgery. Cilostazol may cause dizziness & affect the ability to drive & use of machines.
Adverse Reactions
Headache, diarrhea, abnormal stools & palpitations. Infection; agranulocytosis, anemia, aplastic anemia, bleeding tendency, prolonged bleeding time, ecchymosis, granulocytopenia, leukopenia, pancytopenia, thrombocythemia, thrombocytopenia, increased eosinophils; allergic reaction, anaphylaxis, angioedema; anorexia, DM, edema (peripheral, face), hyperglycemia, hyperuricemia; anxiety; abnormal dreams, cerebral hemorrhage, CVA, dizziness, dull headache, hypoesthesia, insomnia, intracranial hemorrhage, paresis, sleepiness, tremor, vertigo; conjunctivitis, eye hemorrhage, retinal hemorrhage; tinnitus; angina pectoris, arrhythmia, atrial fibrillation, CHF, MI, nodal arrhythmia, supraventricular extrasystoles, supraventricular tachycardia, syncope, tachycardia, Torsades de pointes & QTc prolongation in patients w/ cardiac disorders (eg, complete AV block, heart failure & bradyarrhythmia), ventricular extrasystoles, ventricular tachycardia; epistaxis, hemorrhage (unspecified), hot flushes, HTN, hypotension, orthostatic hypotension, subacute stent thrombosis; cough, dyspnea, interstitial pneumonia, pharyngitis, pneumonia, pulmonary hemorrhage, resp tract hemorrhage, rhinitis; abdominal distention, abdominal pain, dysgeusia, dyspepsia, flatulence, gastritis, GI hemorrhage, heartburn, melena, nausea, vomiting; hepatic dysfunction/abnormal liver function tests, hepatitis, jaundice, increases in: alkaline phosphatase, AST, ALT, lactate dehydrogenase; alopecia, eczema, photosensitivity, pruritus, rash, skin drug eruption (dermatitis medicamentosa), skin eruptions, Stevens-Johnson syndrome, toxic epidermal necrolysis, urticaria, subcutaneous hemorrhage; muscle hemorrhage, myalgia; hematuria, pollakiuria, renal failure, renal impairment; asthenia, chest pain, chills, generalized edema, malaise, pain, pyrexia, sweating, weakness; increases in blood creatinine, blood glucose, BP, BUN, uric acid level; decrease in BP; extradural hematoma, subdural hematoma.
Drug Interactions
Aspirin, other antiplatelet agents, other anticoagulant agents. Erythromycin & other macrolides. Diltiazem. Grapefruit juice. Omeprazole; lovastatin. Ketoconazole, itraconazole, fluconazole, miconazole, fluvoxamine, fluoxetine, nefazodone, sertraline. Carbamazepine, phenytoin, rifampicin, St. John's wort.
