Dactinomycin
Indications/Uses
Wilms' tumor, rhabdomyosarcoma, carcinoma of the testis & uterus, other neoplasms.
Dosage/Direction for Use
Adult 500 mcg IV daily for max 5 days. Childn 15 mcg/kg IV daily for 5 days or 2.5 mg/m2 IV over a 1 wk period. Adult & childn A 2nd course may be given after at least 3 wk if all signs of toxicity have disappeared.
Contraindications
Hypersensitivity. Chickenpox, herpes zoster, severe generalized disease. Pregnancy.
Special Precautions
Hepatic or renal impairment, bone marrow suppression, complicated infections. Do not administer IM or SC. Avoid extravasation, inhalation of dust or vapors or contact w/ skin & mucous membranes. Administration w/in 2 mth of irradiation. Discontinue treatment if stomatitis, diarrhea or severe hemopoietic depression occurs. Monitor renal, hepatic & bone marrow functions regularly. May interfere w/ bioassay procedures for the determination of antibacterial drug levels. Bleeding. Women of childbearing potential. Lactation. Childn.
Adverse Reactions
Alopecia, skin eruptions, acne, erythema flare-up, increased pigmentation of previously irradiated skin; anorexia, nausea, vomiting, abdominal pain, diarrhea, stomatitis, GI ulceration, ascites, hepatomegaly, hepatitis, liver function test abnormalities, jaundice; increased BUN; aplastic anemia, agranulocytosis, leukopenia, thrombocytopenia, pancytopenia, reticulopenia; dyspnea, anaphylaxis; hypernoia, headache, head heaviness, malaise, fatigue, debility, numbness of extremities, convulsion, dizziness; phlebitis; nasal hemorrhage, fever, myalgia, flush, sputum cruentum, hematochezia, compression of pectus, blepharedema, pharyngitis, proctitis, hypocalcemia.
Drug Interactions
Increased effect w/ concomitant radiation therapy or other antineoplastic agents administration. X-ray therapy.
