Indications/Uses
Parenteral nutrition for adults where oral or enteral route for nutrition is not available, not possible, not sufficient or contraindicated.
Dosage/Direction for Use
Individualized dosage. Maintenance of body protein mass Nitrogen requirements: 0.1-0.15 g nitrogen/kg/day in the normal state & degree of catabolic stress. Moderate to high metabolic stress w/ or w/o malnutrition 0.15-0.3 g nitrogen/kg/day. Infusion rate: 1 mL/min through central or peripheral vein for 10-15 min. If no adverse reactions occur, the rest of the soln can be infused at 2-3 mL/min. Infusion period: 12-24 hr.
Contraindications
Hypersensitivity to egg or soy protein. Severe hyperlipidemia, severe liver insufficiency, severe blood coagulation disorders, in born errors of amino acid metabolism, severe renal insufficiency w/o access to hemofiltration or dialysis, acute shock, hyperglycemia which requires > 6 u insulin/h, pathologically elevated serum levels of any of the included electrolytes. General contraindications to infusion therapy: Acute pulmonary HTN, hyperhydration, decompensated cardiac insufficiency & hypotonic dehydration, hemophagocytic syndrome, unstable conditions (eg, severe post-traumatic conditions, uncompensated diabetes, acute MI, metabolic acidosis, severe sepsis & hyperosmolar coma).
Special Precautions
For single use only. Conditions w/ impaired lipid metabolism eg, in renal insufficiency, uncompensated DM, pancreatitis, impaired liver function, hypothyroidism (w/ hypertriglyceridemia) & sepsis; metabolic & lactic acidosis, insufficient cellular O2 supply & increased serum osmolarity; patients w/ a tendency towards electrolyte retention. Disturbances of electrolyte & fluid balance should be corrected before starting the infusion. Do not use if separation of fat emulsion is observed. Should not be given simultaneously w/ blood in the same infusion set due to the risk of pseudo agglutination. Increased risk of infection is associated w/ use of any central vein, strict aseptic precautions should be taken. Monitor serum glucose, electrolytes & osmolarity & fluid balance, acid-base status & liver enzyme tests (alkaline phosphatase, ALT, AST). Monitor blood cell count & coagulation when fat is given for a longer period. Malnourished patients. Pregnancy & lactation. Not suitable for use in newborns or infants <2 yr.
Adverse Reactions
Hyperthermia, sweating, shivering, headache, dysponea. Temporary increased alkaline phosphatase, transaminase & bilirubin levels in long term parenteral nutrition (>6-8 wk). Thrombopenia (in childn). Thrombophlebitis.
Drug Interactions
Some additives eg, electrolytes, trace elements, vit & drugs may be incompatible. Should not be administered w/, before or after an administration of blood through the same equipment due to possibility of pseudo agglutination.
