Cilastatin Na 500 mg, imipenem 500 mg
Indications/Uses
Intra-abdominal, lower resp tract, gynecological, GUT, bone & joint, & skin & soft tissue infections. Septicemia & endocarditis. Mixed infections caused by susceptible strains of aerobic & anaerobic bacteria. Infections caused by aerobic & anaerobic gm +ve & -ve bacteria resistant to cephalosporins, aminoglycosides &/or penicillins.
Dosage/Direction for Use
IV infusion Administer 1,000 mg every 8 hr over 40-60 min & doses 500 mg every 6 hr over 20-30 min. Max total daily dose: 4 g/day. Adult Suspected or proven infection is due to susceptible bacterial spp CrCl ≥90 mL/min 500 mg every 6 hr or 1,000 mg every 8 hr, <90 to ≥60 mL/min 400 mg every 6 hr or 500 mg every 6 hr, <60 to ≥30 mL/min 300 mg every 6 hr or 500 mg every 8 hr, <30 to ≥15 mL/min 200 mg every 6 hr or 500 mg every 12 hr, <15 mL/min undergoing hemodialysis 200 mg every 6 hr or 500 mg every 12 hr. Suspected or proven infection is due to bacterial spp w/ intermediate susceptibility CrCl ≥90 mL/min 1,000 mg every 6 hr, <90 to ≥60 mL/min 750 mg every 8 hr, <60 to ≥30 mL/min 500 mg every 6 hr, <30 to ≥15 mL/min 500 mg every 12 hr, <15 mL/min undergoing hemodialysis 500 mg every 12 hr. Non-CNS infections Ped patient ≥3 mth 15-25 mg/kg every 6 hr, 4 wk-3 mth 25 mg/kg every 6 hr, 1-4 wk 25 mg/kg every 8 hr, <1 wk 25 mg/kg every 12 hr. Max total daily dose: 4 g/day.
Contraindications
Special Precautions
Not indicated for meningitis. Partial cross-allergenicity w/ other β-lactam antibiotics, penicillins & cephalosporins. History of GI disease, particularly colitis. Patients w/ CNS disorders (eg, brain lesions or history of seizures) &/or compromised renal function in whom accumulation of drug could occur. Evaluate neurologically if focal tremors, myoclonus or seizures occur; decrease dose or discontinue if CNS symptoms continue. Do not give to patients w/ CrCl ≤5 mL/min/1.73 m2 unless hemodialysis is instituted w/in 48 hr. Concomitant use w/ valproic acid/divalproex Na is generally not recommended. Pregnancy & lactation. Not recommended in ped patient <3 mth, or w/ impaired renal function (serum creatinine <2 mg/dL).
Adverse Reactions
Erythema, local pain & induration, thrombophlebitis; rash, pruritus, urticaria, erythema multiforme, SJS, angioedema, candidiasis, fever including drug fever, anaphylactic reactions; nausea, vomiting, diarrhea, staining of teeth &/or tongue; eosinophilia, leukopenia, neutropenia including agranulocytosis, thrombocytopenia, thrombocytosis, & decreased Hb, pancytopenia & prolonged prothrombin time, +ve direct Coomb's test; increased serum transaminases, bilirubin &/or serum alkaline phosphatase; oliguria/anuria, polyuria; elevations in serum creatinine & BUN, urine discoloration; paresthesia, encephalopathy, agitation, & dyskinesia; myoclonic activity, psychic disturbances including hallucinations, confusional states, or seizures; hearing loss, taste perversion; drug-related nausea &/or vomiting in granulocytopenic patients.
Drug Interactions
Generalized seizures w/ ganciclovir. Reduced conc of valproic acid or divalproex Na.
