Antithymocyte immunoglobulin (equine)
Indications/Uses
Management of allograft rejection, including delay of onset of 1st rejection episode in patients who have undergone renal transplantation. Adjunct w/ conventional therapy to delay onset of 1st rejection episode. Moderate to severe aplastic anemia in patients not suitable for bone marrow transplant.
Dosage/Direction for Use
Renal allograft Adult 10-30 mg/kg or 10-15 mg/kg daily for 14 days (14 doses) followed by alternate day therapy for further 14 days (7 doses). Bringing the total doses to 21-28 days. Childn 5-25 mg/kg daily. When given to delay onset of 1st rejection episode, start therapy w/in 24 hr before or after transplant. When given to treat rejection, start therapy at the time of diagnosis of the 1st rejection episode. Aplastic Anemia 10-20 mg/kg daily for 8-14 days followed by alternate day therapy for a total of 21 doses.
Contraindications
Previous history of severe systemic reaction to Thymogam or any other equine globulin prep.
Special Precautions
Discontinue therapy if anaphylaxis or severe & unremitting thrombocytopenia or leucopenia occurs. Possibility of transmission of some infectious diseases may occur; cytomegalovirus infection. Recommend skin testing prior commencing treatment for those at greatest risk of systemic anaphylaxis. Monitor patients carefully for concurrent infections. Pregnancy & lactation.
Adverse Reactions
Fever, chills, leucopenia, thrombocytopenia, arthralgia, dermatologic reactions eg, rash, urticaria, wheal, flare & pruritus. Abnormal liver & renal function tests, serum sickness; headache, nausea, vomiting, diarrhea, dyspnea, hypotension, night sweats, stomatitis, chest pain, back pain, pain at the infusion site, clotted A/V fistula & peripheral thrombophlebitis.
