Contents
Per 2 g/250 mg inj Piperacillin Na (equiv to piperacillin) 2 g, tazobactam Na (equiv to tazobactam) 250 mg. Per 4 g/500 mg inj Piperacillin Na (equiv to piperacillin) 4 g, tazobactam Na (equiv to tazobactam) 500 mg
Indications/Uses
Multidrug-resistant gm -ve infections due to organisms proven or suspected to be susceptible to piperacillin-tazobactam except CNS infections; polymicrobial infections (eg, mixed aerobic & anaerobic infections) in which other agents have insufficient activity or are contraindicated due to toxic potential.
Dosage/Direction for Use
Adult & adolescent Severe pneumonia; neutropenia w/ fever suspected to be due to a bacterial infection; complicated UTI (including pyelonephritis) 4 g/500 mg every 6 hr. Complicated intra-abdominal, skin & soft tissue (including diabetic foot) infections 4 g/500 mg every 8 hr. Childn 2-12 yr Neutropenia w/ fever suspected to be due to bacterial infections Piperacillin 80 mg/tazobactam 10 mg/kg every 8 hr. Max: 4 g/500 mg/dose over 30 min. Complicated intra-abdominal infections Piperacillin 100 mg/tazobactam 10 mg/kg every 8 hr. Max: 4 g/500 mg/dose over 30 min.
Contraindications
Hypersensitivity to piperacillin, tazobactam & other penicillins. History of acute severe allergic reaction to any other β-lactams (eg, cephalosporin, monobactam or carbapenem).
Special Precautions
Discontinue use if serious hypersensitivity reactions, pseudomembranous colitis symptoms or bleeding manifestations occur. Perform periodic assessment of hematopoietic function. Risk of superinfections; neurological complications in the form of convulsions (high doses especially in patients w/ renal impairment). Patients on a controlled Na diet. Perform periodic electrolyte determinations in patients w/ low K reserves or those receiving concomitant drugs that may lower K levels. Childn <2 yr.
Adverse Reactions
Diarrhea, vomiting, nausea; rash including maculopapular rash.
Drug Interactions
Prolongation of the neuromuscular blockade of vecuronium & any nondepolarizing muscle relaxant. Heparin, oral anticoagulants & other drugs that may affect the blood coagulation system including thrombocyte function. May reduce excretion of MTX. Longer t½ & lower renal clearance w/ probenecid.
