Indications/Uses
Susceptible infections especially those due to Pseudomonas spp including biliary tract, urinary tract, resp tract (cystic fibrosis), skin & soft tissue (including burns, ecthyma gangrenosum & ulceration) infections; pneumonia, ENT, endophthalmitis, meningitis, septicaemia, & infections in immunocompromised patients.
Dosage/Direction for Use
Dose can be given by deep IM inj, slow IV inj over 3-5 min or IV infusion over 30 min. Adult 1-6 g daily in divided doses every 8-12 hr. Cystic fibrosis w/ pseudomonal lung infections 100-150 mg/kg daily in 3 divided doses. Cystic fibrosis w/ normal renal function Up to 9 g/day; >1 g single dose should be given IV. Childn 30-100 mg/kg daily in 2-3 divided doses. Severely ill childn 150 mg/kg daily. Max: 6 g daily in 3 divided doses. Neonate & infant ≤2 mth 25-60 mg/kg daily in 2 divided doses. Elderly Max: 3 g daily. Renal impairment CrCl 31-50 mL/min 1 g every 12 hr, 16-30 mL/min 1 g every 24 hr, 16-6 mL/min 0.5 g every 24 hr, <5 mL/min 0.5 g every 48 hr. Patient undergoing peritoneal dialysis Loading dose: 1 g, followed by 500 mg every 24 hr. Patients on hemodialysis Repeat appropriate maintenance dose after each dialysis period. Prophylaxis in prostatic surgery 1 g at induction of anesth, repeat if necessary.
Special Precautions
Potential for colonization & superinfection w/ resistant organisms. Renal insufficiency.
Adverse Reactions
Drug Interactions
Enhanced activity against Pseudomonas aeruginosa & Enterobacteriaceae w/ aminoglycosides. Nephrotoxicity & ototoxicity w/ aminoglycosides or potent diuretics eg, furosemide. Antagonism w/ chloramphenicol.
