Indications/Uses
Prophylaxis of organ rejection in patients receiving allogeneic liver, kidney, or heart transplants.
Dosage/Direction for Use
Liver transplantation Initially 0.10-0.15 mg/kg/day every 12 hr in 2 divided doses. Administer initial dose no sooner than 6 hr after transplantation. If receiving IV infusion, 1st dose of oral therapy should be given 8-12 hr after discontinuing IV infusion. Kidney transplantation Initially 0.2 mg/kg/day in combination w/ azathioprine or 0.1 mg/kg/day in combination w/ MMF & IL-2 receptor antagonist. May administer w/in 24 hr of transplantation. Delay until renal function has recovered. Patient w/ heart transplantation Initially 0.075 mg/kg/day every 12 hr in 2 divided doses. Administer initial dose no sooner than 6 hr after transplantation. If receiving IV infusion, 1st dose of oral therapy should be given 8-12 hr after discontinuing IV infusion. Childn Initially 0.15-0.20 mg/kg/day.
Administration
Should be taken on an empty stomach.
Contraindications
Hypersensitivity to tacrolimus & to polyoxyl 60 hydrogenated castor oil.
Special Precautions
HTN. Avoid K-sparing diuretics. Risk of malignancy (non-Hodgkin's lymphoma & carcinoma of the skin). Renal or hepatic impairment. Pregnancy & lactation.
Adverse Reactions
Tremor, headache, diarrhea, HTN, abnormal renal function, infection. Liver transplantation: Nausea, hyperkalemia & hypomagnesemia. Kidney transplantation: Constipation, abdominal pain & insomnia. Heart transplantation: DM, CMV infection, hyperglycemia, leukopenia, & hyperlipemia.
Drug Interactions
Avoid use w/ live vaccines. Higher exposure w/ mycophenolic acid. Increased blood trough conc in liver transplant patients w/ grapefruit juice.
