Desflurane
Indications/Uses
Inhalation agent for induction of anesth for inpatient & outpatient surgery in adults; & for maintenance of anesth for inpatient & outpatient surgery in adults & in ped patients.
Dosage/Direction for Use
Induction Adult Premed w/ opioid. Frequent starting conc is 3%, increased in 0.5-1% increments every 2-3 breaths. Maintenance Adult 2.5-8.5% w/ or w/o concomitant use of nitrous oxide. Childn 5.2-10% w/ or w/o concomitant use of nitrous oxide.
Contraindications
Sensitivity to desflurane or to other halogenated agents. Known or suspected genetic susceptibility to malignant hyperthermia. Patient in whom general anesth is contraindicated. History of moderate to severe hepatic dysfunction. Induction of anesth in ped patients.
Special Precautions
Not to be used as sole agent for anesth induction in patients w/ CAD or patients where increases in heart rate or BP are undesirable. May produce dose-dependent increase in CSF pressure in patients w/ intracranial space occupying lesions. Discontinue use if malignant hyperthermia occurs. Early & aggressive intervention to treat hyperkalemia & resistant arrythmias is recommended, as is subsequent evaluation for latent neuromuscular disease. Not for maintenance of anesth in non-intubated childn. Sensitivity hepatitis in patients who have been sensitized by previous exposure to halogenated anesth. Ped neurotoxicity. Post-op agitation in childn. Closely monitor childn ≤6 yr or w/ asthma or history of recent upper airway infection for signs & symptoms associated w/ laryngospasm or airway narrowing. Monitor cardiac rhythm in patients w/ congenital long QT syndrome or those taking drugs that can prolong the QT interval. Can react w/ desiccated CO2 absorbents to produce carbon monoxide which may result in elevated levels of carboxyhemoglobin. Transient elevations in glucose & WBC count may occur. Pregnancy & lactation. Elderly.
Adverse Reactions
Induction in adults: Coughing, breath holding, apnea, increased secretions, laryngospasm, oxyhemoglobin desaturation, pharyngitis. Maintenance/recovery in adults & intubated ped patients: Headache; bradycardia, HTN, nodal arrhythmia, tachycardia; nausea, vomiting; increased salivation; apnea, breath holding, increased cough, laryngospasm, pharyngitis; conjunctivitis (conjunctival hyperemia).
Drug Interactions
MAC reduction w/ benzodiazepines & opioids; & nitrous oxide. Reduced ED95 of succinylcholine, atracurium & pancuronium.
