Indications/Uses
Management of hyperlipidaemias including primary hypercholesterolaemia (type IIa), mixed dyslipidaemia (type IIb), & hypertriglyceridaemia (type IV). Homozygous familial hypercholesterolaemia.
Dosage/Direction for Use
Initially 5-10 mg in the evening. Patients w/ ischaemic heart disease Initially 20 mg, may adjust the dose at not <4 wk interval up to max of 80 mg once daily in 3 divided doses (20 mg, 20 mg, & evening dose of 40 mg). Patients taking cyclosporine, fibric acid derivatives, or nicotinic acid Max: 10 mg daily.
Administration
May be taken with or without food.
Contraindications
Hypersensitivity. Active liver disease or unexplained persistent elevations of serum transaminases. Pregnancy & lactation.
Special Precautions
Patients w/ history of liver disease &/or those who consume substantial quantities of alcohol. Patients w/ predisposing factors for myopathy eg, renal impairment, advanced age & inadequately treated hypothyroidism. Uncomplicated myalgia & myopathy. Advise patients to promptly report unexplained muscle pain, tenderness, or weakness accompanied by malaise or fever. Discontinue therapy if markedly elevated CK levels occur or myopathy is diagnosed or suspected. Temporarily w/held in patient w/ acute, serious condition suggestive of myopathy or predisposing to the development of renal failure secondary to rhabdomyolysis (eg, sepsis, hypotension, dehydration, major surgery, trauma, severe metabolic endocrine, & electrolyte disorders, or uncontrolled seizures). Concomitant use w/ other lipid lowering therapies, cyclosporine, or lopinavir/ritonavir. Avoid combination w/ gemfibrozil.
Adverse Reactions
GI disturbances, headache, skin rashes, dizziness, blurred vision, insomnia & dysgeusia.
Drug Interactions
Increased conc w/ cyclosporine or gemfibrozil. Increased risk of myopathy w/ other lipid lowering therapies, cyclosporine, or lopinavir/ritonavir.
