Per mL Bupivacaine HCl 5 mg, dextrose monohydrate 80 mg
Indications/Uses
Infiltration anesth & regional nerve blocks, particularly epidural & peripheral nerve block. Epidural analgesia in labor & delivery.
Dosage/Direction for Use
Adult Spinal anesth for surgery 1.5-3 mL (7.5-15 mg) & 2-4 mL (10-20 mg). Lower limb, urological & perineal surgery 1.5-3 mL (7.5-15 mg) on L3/4/5 site of inj, sitting position. Lower abdominal operations (including caesarean section) 3-4 mL (15-20 mg) or 2-4 mL (10-20 mg) on L2/3/4 site of inj, horizontal position. Childn >15 kg 0.25-0.3 mg/kg, 5-15 kg 0.3-0.4 mg/kg. Infant ≤5 kg 0.4-0.5 mg/kg.
Contraindications
Hypersensitivity to local anesth of amide type. Acute active diseases of the CNS eg, meningitis, poliomyelitis, intracranial hemorrhage, demyelinating, increased intracranial pressure, cerebral & spinal tumors. Spinal stenosis & active disease (eg, spondylitis, tumor) or recent trauma (eg, fracture) in the vertebral column. TB of the spine. Pyrogenic infection of the skin at or adjacent to the lumbar puncture site. Septicemia. Pernicious anemia w/ subacute combined degeneration of the spinal cord. Coagulation disorders or ongoing anticoagulation treatment. Uncorrected hypotension, cardiogenic or hypovolemic shock. Obstet paracervical block, IV regional anesth (Bier's block) & all IV infusions.
Special Precautions
Discontinue w/ appearance of signs of, acute systemic toxicity or total spinal block. Patients w/ cardiac block, impaired liver function & CV disturbances. May have a very mild effect on mental function & coordination. Patients w/ pleural effusions may suffer resp embarrassment. Possibility of hypotension & bradycardia following epidural or subarachnoid blockade, including prior establishment of IV line & the availability of vasopressor agents & oxygen. Hypotension. Patients w/ genetic predisposition to malignant hyperthermia. Pregnancy (increased risk during the late stages) & lactation. Elderly.
Adverse Reactions
Hypotension, bradycardia, post-dural puncture headache, CV & resp depression. Occasionally permanent localized paresthesia or anaesth, motor weakness, loss of sphincter control or paraplegia. Potential acute toxic effects on CNS & CVS.
Drug Interactions
Increased risk of myocardial depression w/ antiarrhythmic drugs. Increased risk of bupivacaine HCl toxicity w/ propranolol.
