Indications/Uses
Sedation of initially intubated & mechanically ventilated patients during treatment in an intensive care setting. Sedation/anxiolysis of non-intubated patients prior to &/or during surgical & other procedures.
Dosage/Direction for Use
Individualized & titrated to desired clinical response. Infusion should not exceed 24 hr. ICU sedation Adult Initiation: 1 mcg/kg loading infusion over 10 min. Maintenance: 0.2-0.7 mcg/kg/hr. Adjust rate of maintenance infusion to achieve the desired level of sedation. Procedural sedation Adult Initiation: 1 mcg/kg loading infusion over 10 min. 0.5 mcg/kg loading infusion over 10 min for less invasive procedure eg, ophth surgery. Maintenance: Initially 0.6 mcg/kg/hr & titrate w/ doses ranging from 0.2-1 mcg/kg/hr. Adjust rate of maintenance infusion to achieve the target level of sedation. Awake fiberoptic intubation in adult patient Initiation: 1 mcg/kg loading infusion over 10 min. Maintenance: 0.7 mcg/kg/hr until the endotracheal tube is secured. Patient >65 yr Initiation: 0.5 mcg/kg loading infusion over 10 min.
Special Precautions
Discontinue use if drug-related hyperthermia or pyrexia is suspected. Patients w/ advanced heart block &/or severe ventricular dysfunction. Hypotension &/or bradycardia may be more pronounced in patients w/ hypovolemia, DM or chronic HTN. Transient HTN primarily during the loading dose. Arousability & alertness when stimulated has been observed in some patients. Supportive therapy is indicated if tachycardia &/or HTN occurs after discontinuation in ICU sedation. Tolerance & tachyphylaxis w/ use beyond 24 hr. Clonidine-like w/drawal syndrome upon abrupt discontinuation. Hepatic impairment. Pregnancy (category C) & lactation. Childn. Elderly ≥65 yr.
Adverse Reactions
Hypotension, bradycardia & sinus arrest; dry mouth. Nausea, fever, vomiting, hypoxia, tachycardia & anemia.
Drug Interactions
Enhanced effects w/ anesth, sedatives, hypnotics & opioids.
