Indications/Uses
1st-line therapy of patients w/ locally advanced or metastatic nonsquamous NSCLC in combination w/ cisplatin. Monotherapy for maintenance of patients w/ nonprogressive locally advanced or metastatic nonsquamous NSCLC after 4 cycles 1st-line chemotherapy w/ platinum; for patients w/ locally advanced or metastatic nonsquamous NSCLC after prior chemotherapy. Patients w/ malignant pleural mesothelioma not eligible for surgery in combination w/ cisplatin.
Dosage/Direction for Use
IV infusion Malignant pleural mesothelioma & nonsquamous NSCLC in combination w/ cisplatin 500 mg/m2 administered over 10 min on day 1 of each 21-day cycle. Recommended cisplatin dose: 75 mg/m2 over >2 hr infusion time approx 30 min after finishing Pemex IV infusion on the 1st day of 21-day cycle. Nonsquamous NSCLC monotherapy 500 mg/m2 administered over 10 min on day 1 of each 21-day cycle.
Contraindications
History of severe hypersensitivity reaction. Concomitant use w/ yellow fever vaccine.
Special Precautions
Premed w/ folate & vit B12; corticosteroid (dexamethasone) supplementation. Bone marrow suppression. Not to be administered in patients w/ CrCl <45 mL/min. Concomitant use w/ NSAIDs (including ibuprofen) in patients w/ mild to moderate renal insufficiency (CrCl 45-79 mL/min). Do not begin a new cycle of treatment unless ANC is ≥1,500 cells/mm3, platelet count is ≥100,000 cells/mm3 & CrCl is ≥45 mL/min. Consider human sperm preservation before treatment. Patients should receive adequate antiemetic treatment & appropriate hydration prior to &/or after treatment. CV risk. Use of radical sensitizer in patients w/ radiotherapy before, during or after treatment. Renal impairment. Use effective contraceptive measures during treatment in women of childbearing potential. Male patients should not father child during or w/in 6 mth after treatment; take contraceptive measures or avoid sexual activities. Pregnancy & lactation. Childn. Elderly.
Adverse Reactions
Fatigue, nausea & anorexia. Vomiting, neutropenia, leukopenia, anemia, stomatitis/pharyngitis, thrombocytopenia & constipation.
Drug Interactions
Decreased clearance w/ ibuprofen. Concomitant use w/ high dose of NSAIDs or aspirin; NSAIDs w/ short elimination or longer t½. Delayed clearance w/ nephrotoxic drugs (eg, aminoglycoside, loop diuretics, platinum compd, cyclosporine); substances that are tubularly secreted (eg, probenecid). May require increased INR monitoring frequency w/ oral anticoagulants. Risk of fatal general disease w/ attenuated live vaccine.
