GERD, peptic ulcer, for eradication of H. pylori (may be combined w/ 2 antibacterial in a 1-wk triple therapy regimen). Prophylaxis for NSAID-associated ulceration & treatment of pathological hypersecretory states eg, Zollinger-Ellison syndrome.
GERD 20-40 mg once daily for 4 wk, increased to 8 wk if necessary. For maintenance therapy, may be continued w/ 20-40 mg daily. PUD 40 mg once daily given for 2-4 wk for duodenal ulceration, or 4-8 wk for benign gastric ulceration. H. pylori eradication May be combined w/ 2 antibacterials in a 1-wk triple therapy regimen. Regimens may include pantoprazole 40 mg bid combined w/ clarithromycin 500 mg bid & either amoxicillin 1 g bid or metronidazole 400 mg bid.
May mask symptoms & delay diagnosis of possibility of malignancy. Regularly monitor liver function. Not for mild GI complaints eg, nervous dyspepsia. Patient w/ reduced body stores or risk factors for reduced vit B12 absorption on long-term therapy. Hepatic impairment. Pregnancy & lactation. Not recommended in childn <12 yr.
May alter metabolism of some other drugs metabolized by CYP450 system, primarily by isoenzyme CYP2C19. May prolong elimination of diazepam, phenytoin & warfarin. May reduce absorption of drugs eg, ketoconazole, & possibly itraconazole, whose absorption is dependent on acid gastric pH.
