Indications/Uses
Prevention of relapse in patients w/ reflux esophagitis. Short-term symptomatic relief & healing of active duodenal & benign gastric ulcer, GERD & history of erosive esophagitis. In combination w/ clarithromycin & either amoxicillin or metronidazole for active duodenal ulcer in H. pylori +ve patients. Pathological hypersecretory conditions associated w/ Zollinger-Ellison syndrome in adults.
Dosage/Direction for Use
Adult GERD associated w/ history of erosive esophagitis 40 mg once daily by IV infusion for 7-10 days. Pathological hypersecretion including Zollinger-Ellison syndrome 80 mg IV every 12 hr. Renal impairment Max: 40 mg daily. Hepatic impairment Max: 20 mg daily or 40 mg on alternate days.
Contraindications
Special Precautions
Anaphylaxis & other serious reactions eg, erythema multiforme, SJS & TEN. Symptomatic response to therapy does not preclude the presence of gastric malignancy. Thrombophlebitis. Increased risk of C. difficile-associated diarrhea & osteoporosis-related fractures of the hip, wrist or spine. Mild, transient transaminase elevations & hypomagnesemia. May produce false +ve urine screen for tetrahydroxycannabinol. Concomitant use w/ MTX. Pregnancy & lactation. Childn. Elderly ≥65 yr.
Adverse Reactions
Abdominal & chest pain, rash, pruritus. Headache, inj site reaction, dyspepsia, diarrhea, vomiting, dizziness, rhinitis; angioedema (Quincke's edema); anterior ischemic optic neuropathy; severe dermatologic reactions including erythema multiforme, SJS & TEN; hepatocellular damage leading to jaundice & hepatic failure; pancreatitis, pancytopenia, rhabdomyolysis; confusion, hypokinesia, speech disorder, increased salivation, vertigo, nausea, tinnitus & blurred vision.
Drug Interactions
Decreased atazanavir or nelfinavir plasma conc. Increased INR & prothrombin time w/ warfarin/coumarin anticoagulants. May interfere w/ absorption of drugs (including ketoconazole, ampicillin esters, Fe salts & digoxin). May elevate & prolong serum levels of MTX &/or hydroxymethotrexate.
