Pantoprazole Na sesquihydrate
Indications/Uses
In conditions where inhibition of gastric acid secretion may be beneficial, including syndromes, dyspepsia, GERD, PUD, & Zollinger-Ellison syndrome.
Dosage/Direction for Use
GERD 20-40 mg once daily for 4 wk. Dyspepsia 10-20 mg once daily for 2-4 wk. PUD 20 mg as single daily dose or 40 mg in severe cases. NSAID-associated ulceration 20 mg daily. Zollinger-Ellison syndrome 40 mg once daily. Acid aspiration prophylaxis during general anesth 40 mg in the evening before surgery, & a further 40 mg 2-6 hr before the procedure.
Administration
May be taken with or without food: Swallow whole, do not chew/crush/split.
Contraindications
Hypersensitivity to pantoprazole or substituted benzimidazoles. Co-administration w/ rilpivirine.
Special Precautions
Consider the possibility of malignancy since it may mask symptoms & delay diagnosis in patients w/ gastric ulcer. Pseudomembranous colitis w/ antibiotic combination therapy. Increased risk of GI infections; consider diagnosis of Clostridium difficile infection & Clostridium difficile-associated diarrhea for diarrhea that does not improve. Increased risk for osteoporosis-related fractures of the hip, wrist, or spine (w/ high doses & long-term therapy). Hypergastrinemia, possible enterochromaffin-like cell hyperplasia & carcinoid formation in the stomach, adenomas & carcinomas in the liver & neoplastic changes in the thyroid w/ long term treatment. Concomitant use w/ MTX. Hepatic impairment. Pregnancy & lactation.
Adverse Reactions
Headache, diarrhea & skin rashes. Pruritus, fatigue, constipation, nausea & vomiting, flatulence, abdominal pain, arthralgia & myalgia, urticaria & dry mouth; GI infections.
Drug Interactions
Potentiated effect on the elimination rate of H. pylori infection w/ tetracycline, clarithromycin, & amoxicillin. Significant reduction in the bioavailability of HIV PIs eg, atazanavir. May elevate & prolong serum levels of MTX (primarily at high dose) &/or its metabolite.
