Indications/Uses
Management of nausea & vomiting induced by cytotoxic chemotherapy & RT. Prevention & treatment of post-op nausea (PONV) & vomiting. Management of chemotherapy-induced nausea & vomiting (CINV) in childn ≥6 mth & for the prevention & treatment of PONV in childn ≥1 mth.
Dosage/Direction for Use
Tab Prevention of acute emesis w/ less emetogenic chemotherapy &/or RT 8 mg up to 2 hr before treatment, followed by 8 mg 8-12 hr later. Prevention of post-op nausea & vomiting Adult 16 mg 1 hr before anesth or 8 mg 1 hr before anesth followed by 2 further doses of 8 mg at 8-hr intervals. Patient w/ hepatic impairment Max: 8 mg daily. Inj Prevention of acute emesis w/ highly emetogenic chemotherapy 8 mg by slow IV or IM inj immediately before treatment, either followed by continuous IV infusion of 1 mg/hr for up to 24 hr, or by further 2 doses of 8 mg 2-4 hr apart. Prevention of post-op nausea & vomiting Adult Single dose of 4 mg IM or slow IV inj at induction of anesth. Treatment of post-op nausea & vomiting Single dose of 4 mg IM or slow IV inj. Patient w/ moderate or severe hepatic impairment Max: 8 mg daily.
Administration
May be taken with or without food.
Contraindications
Special Precautions
Hypersensitivity. Patients w/ congenital long QT syndrome. Hypokalaemia & hypomagnesaemia should be corrected prior to therapy. Serotonin syndrome. Monitor for signs of sub-acute intestinal obstruction. May mask occult bleeding in patients w/ adenotonsillar surgery. Galactose intolerance, Lapp lactase-deficiency or glucose-galactose malabsorption. Pregnancy & lactation. Childn.
Adverse Reactions
Headache. Sensation of warmth or flushing; constipation. Inj: Local IV inj site reactions.
Drug Interactions
Increase risk of arrhythmias w/ cardiotoxic drugs eg, anthracyclines or trastuzumab, erythromycin, ketoconazole, amiodarone & atenolol or timolol. Serotonin syndrome w/ SSRIs & SNRIs. Profound hypotension & loss of consciousness w/ apomorphine. Decreased blood conc w/ phenytoin, carbamazepine & rifampicin. Reduced analgesic effect of tramadol.
