Epirubicin HCl
Indications/Uses
Treatment of a wide range of neoplastic conditions including breast, ovarian, gastric, lung, liver & colorectal carcinomas, malignant lymphomas, leukemia, multiple myeloma & soft tissue sarcoma. Superficial bladder cancer (carcinoma-in-situ). Prophylaxis of recurrences after transurethral resection.
Dosage/Direction for Use
IV inj in a soln of NaCl 0.9% or water for inj into a fast-running infusion of NaCl 0.9% or glucose 5% over 3-5 min or by infusion over up to 30 min. Usual dose: 60-90 mg/m2 as a single dose every 3 wk. May be divided over 2-3 days. Superficial bladder cancer 50 mg wkly as 0.1% soln for 8 wk reduced to 30 mg wkly. Increase to 80 mg wkly if chemical cystitis develops for carcinoma in-situ. Prophylaxis of recurrence in patient who have undergone transurethral resection 50 mg wkly for 4 wk followed by 50 mg instilled once a mth for 11 mth.
Contraindications
Hypersensitivity. Current or previous history of cardiac impairment, patients already treated w/ maximal cumulative doses of other anthracyclines eg, epirubicin or daunorubicin, patient w/ heart disease. Pregnancy & lactation.
Special Precautions
Hepatic & renal dysfunction, myelosuppression, infectious complication, chickenpox, heart failure. Careful baseline assessment of blood counts; serum levels of total bilirubin, AST & creatinine; cardiac function should be done prior to treatment. Cardiac toxicity. Promptly discontinue treatment at the 1st sign of impaired cardiac function. Infections & bleeding tendency. Do not administer as IM or SC. Avoid intraperitoneal prescribing. Childn. Elderly.
Adverse Reactions
Supraventricular tachycardia, arrhythmia, abdominal pain, ECG changes; anemia, bleeding, leukocytopenia & thrombopenia; fever, chills & urticaria, anaphylaxis; hepatic impairment; proteinuria; acute nausea & vomiting; mucositis, anorexia & diarrhea. Alopecia; fatigue & headache; oliguria, urodynia, urocystitis, hematuria; pneumothorax; febricity, facial edema.
Drug Interactions
Other cytotoxic drugs, Ca-channel blockers, radiation therapy.
