Nebivolol HCl
Indications/Uses
Treatment of essential HTN & stable mild & moderate chronic heart failure in addition to standard therapies in elderly patients.
Dosage/Direction for Use
HTN Adult 5 mg once daily. In combination w/ other antihypertensive agents 5 mg plus hydrochlorothiazide 12.5-25 mg. Elderly >65 yr Start at 2.5 mg daily. May be increased to 5 mg daily. Chronic heart failure Initially 1.25 mg once daily, increase at interval of 1-2 wk based on patient's tolerability to 2.5, 5 & 10 mg once daily.
Administration
May be taken with or without food: Preferably taken at the same time each day.
Contraindications
Hypersensitivity. Liver insufficiency or impairment. Acute heart failure, cardiogenic shock or episodes of heart failure decompensation. Sick sinus syndrome including SA block; 2nd- & 3rd-degree heart block (w/o a pacemaker); history of bronchospasm & bronchial asthma; untreated pheochromocytoma; metabolic acidosis; bradycardia (heart rate <60 bpm); hypotension (systolic BP <90 mmHg); severe peripheral circulatory disturbances.
Special Precautions
Concomitant use w/ anesth. Untreated CHF, ischemic heart disease. Peripheral circulatory disorders (Raynaud's disease or syndrome, intermittent claudication); 1st-degree heart block; Prinzmetal's angina. DM, hyperthyroidism, COPD, history of psoriasis. May increase sensitivity to allergens & severity of anaphylactic reactions. Avoid abrupt w/drawal. Severe renal impairment. Galactose intolerance, Lapp lactase deficiency or glucose-galactose malabsorption. May impair ability to drive or operate machinery. Pregnancy & lactation. Childn & adolescent. Elderly >75 yr.
Adverse Reactions
Headache, dizziness, paresthesia; dyspnea; constipation, nausea, diarrhea; tiredness, edema; bradycardia.
Drug Interactions
Class I antiarrhythmics & amiodarone, verapamil or diltiazem type Ca antagonist, centrally-acting antihypertensives, anesth, insulin & oral antidiabetic drugs. Digitalis glycosides; dihydropyridines; TCAs, barbiturates, phenothiazines, sympathicomimetic agents. CYP2D6 inhibitors, cimetidine, nicardipine.
