Indications/Uses
Duodenal & gastric ulcer, reflux oesophagitis, & Zollinger-Ellison syndrome. H. pylori-associated duodenal ulcer w/ suitable antibiotics.
Dosage/Direction for Use
Duodenal ulcer 20 mg once daily for 2-4 wk. Patient w/ duodenal ulcer refractory to other treatment regimens 40 mg once daily may be effective. Maintenance: 10-20 mg once daily. Gastric ulcer & reflux oesophagitis 20 mg once daily for 4-8 wk. Patient w/ gastric ulcer or reflux oesophagitis refractory to other treatment regimens 20 mg once daily may be effective. Patient w/ severe or symptomatic recurrent reflux oesophagitis Continue treatment at 20 mg once daily. Zollinger-Ellison syndrome Initially 60 mg once daily. Patient w/ severe disease Maintenance: 20-120 mg daily. Doses >80 mg daily should be divided into bid regimens. Hepatic impairment 20 mg daily.
Administration
Should be taken on an empty stomach: Take at least 1 hr before meals. Swallow whole, do not chew/crush. For patients w/ difficulty swallowing, cap may be carefully opened & entire contents sprinkled in a spoonful of applesauce. Swallow drug/food mixt w/o chewing immediately after prep. Drug/food mixt should not be stored for future use.
Contraindications
Hypersensitivity. Pregnancy & lactation.
Special Precautions
Exclude possibility of malignancy if gastric ulcer is suspected as treatment may mask the symptoms & delay diagnosis. Childn.
Adverse Reactions
Nausea/vomiting, headache, diarrhoea, constipation, abdominal pain & flatulence, rash &/or pruritus, dizziness, paraesthesia, somnolence, insomnia, vertigo, increased liver enzymes & malaise.
Drug Interactions
Prolonged elimination of medicines metabolised by oxidation in the liver eg, warfarin, diazepam & phenytoin. May increase plasma conc of clarithromycin. Increased bioavailability of digoxin.
