Metoclopramide HCl
Indications/Uses
Disorders of decreased GI motility eg, gastroparesis or ileus; GERD & dyspepsia; nausea & vomiting associated w/ various GI disorders, migraine, post-op & cancer therapy. Stimulate gastric emptying during radiographic exam, facilitate intubation of small bowel, management of aspiration syndromes.
Dosage/Direction for Use
Adult & childn 15-19 yr & weighing ≥60 kg 10 mg tid; 15-19 yr, weighing 30-59 kg & 9-14 yr (≥30 kg) 5 mg tid; 5-9 yr (20-29 kg) 2.5 mg tid; 3-5 yr (15-19 kg) 2 mg bid-tid; 1-3 yr (10-14 kg) 1 mg bid-tid; <1 yr (≤10 kg) 1 mg bid. Nausea & vomiting associated w/ cancer chemotherapy Loading dose: 2-4 mg/kg as continuous IV infusion over 15-20 min, followed by a maintenance dose of 3-5 mg/kg over 8-12 hr. Alternative: Initially up to 2 mg/kg by IV infusion over at least 15 min, repeated every 2-3 hr. Max: 10 mg/kg/day, either continuous or intermittent infusion.
Special Precautions
Stimulation of muscular contraction in GI hemorrhage, obstruction, perforation or immediately after surgery. Hypertensive crisis in pheochromocytoma. Epilepsy, Parkinson's disease, history of depression. Review regularly patients in prolonged therapy. May affect ability to drive or operate machinery. Renal impairment. Childn. Elderly.
Adverse Reactions
Extrapyramidal symptoms as acute dystonic reactions (in young adult & childn, especially if female). Parkinsonism & tardive dyskinesia in elderly on prolonged therapy. Restlessness, drowsiness, diarrhea; hypotension, dizziness, headache, depression; blood disorders, hypersensitivity reactions, neuroleptic malignant syndrome; cardiac conduction disorders; galactorrhea or related disorders. Transient increases in plasma aldosterone conc.
Drug Interactions
May cause extrapyramidal reactions w/ phenothiazines. Centrally acting drugs eg, antiepileptics. Increased toxicity w/ lithium. Antagonized GI effects w/ antimuscarinics & opioid analgesics. Reduced absorption of digoxin. Inhibits serum cholinesterase. Prolonged neuromuscular blockade of suxamethonium & mivacurium. Interferes w/ hypoprolactinemic effect of bromocriptine.
