Gentamicin sulfate
Indications/Uses
Treatment of severe systemic infections due to sensitive gm -ve & other organisms including biliary tract infections (acute cholecystitis or cholangitis), brucellosis, cat scratch disease, cystic fibrosis, endocarditis (treatment & prophylaxis of endocarditis due to streptococci, enterococci or staphylococci), endometritis, gastroentiritis, granuloma inguinale, listeriosis, meningitis, otitis externa & media, pelvic inflammatory disease, peritonitis, plague, pneumonia, septicemia, skin infection eg, in burns or ulcers (given systemically for pseudomonal & other gm -ve infections), tularemia & UTI (acute pyelonephritis). Prophylaxis of surgical infection & treatment of immunocompromised patients & those in intensive care.
Dosage/Direction for Use
IM/IV Total daily dose: 3-5 mg/kg divided 8 hrly. Prophylaxis & treatment of streptococcal & enterococcal endocarditis 80 mg bid or 120 mg before anesth induction. UTI 160 mg once daily. Childn 1.5-2 mg/kg 8 hrly. Infant & neonate 2.5 mg/kg 8 hrly or 3 mg/kg 12 hrly. 1st wk of life 2.5 mg/kg 12 hrly. Duration of treatment: 7-10 days.
Contraindications
Hypersensitivity to gentamicin & other aminoglycosides.
Special Precautions
Monitor patients w/ renal impairment, cystic fibrosis, receiving high doses or prolonged courses, obese, infants & elderly. Myasthenia gravis, parkinsonism & other conditions characterized by muscular weakness. Contains Na metabisulfite causing allergic-type reactions. Pregnancy.
Adverse Reactions
Irreversible ototoxicity, vestibular damage, reversible nephrotoxicity, acute renal failure, acute tubular necrosis, interstitial nephritis, electrolyte disturbances, resp depression, muscular paralysis, hypersensitivity reactions, neurotoxicity, atrophy or fat necrosis at inj sites.
Drug Interactions
Increased risk of toxicity w/ other nephrotoxic drugs including other aminoglycosides, vancomycin, cephalosporins, ciclosporin, cisplatin, fludarabine or potentially ototoxic drugs eg, ethacrynic acid & furosemide. Early symptoms of vestibular toxicity may be masked w/ antiemetics eg, dimenhydrinate. Neuromuscular blockers. Risk of severe resp depression w/ general anesth or opioids. Reduced antibacterial effects w/ bacteriostatic antibacterials. May reduce renal excretion of zalcitabine. Synergistic activity w/ β-lactams. May inhibit α-galactoridase activity of α- or β-agalsidase.
