Medroxyprogesterone acetate
Indications/Uses
Contraception & treatment of menorrhagia, secondary amenorrhea, & mild to moderate endometriosis.
Dosage/Direction for Use
IM Contraception & menorrhagia 150 mg/mL w/in the 1st 7 days of cycle or w/in the 1st 4 wk after giving birth (delay until 6 wk after giving birth if breastfeeding). Secondary amenorrhea 5-10 mg daily for 5-10 days until 2 days before anticipated onset of menstruation. Mild to moderate endometriosis 50 mg wkly or 100 mg every 2 wk for at least 6 mth.
Contraindications
Undiagnosed vag bleeding, history or current high risk of arterial disease, hormone-dependent neoplasms, porphyria, after recent evacuation of hydatidiform mole. Hepatic impairment (especially if severe). Pregnancy.
Special Precautions
Cardiac impairment, epilepsy, asthma, history of depression, DM, HTN, migraine, thromboembolism, other conditions aggravated by fluid retention, conditions that cause significant decrease in bone mineral density leading to osteoporotic fractures. Renal impairment. Past ectopic pregnancy. Breastfeeding women should wait 6 wk after giving birth.
Adverse Reactions
GI disturbances, appetite or wt changes, fluid retention, edema, acne, chloasma (melasma), allergic skin rashes, urticaria, mental depression, breast changes including discomfort or gynecomastia (occasionally), changes in libido, hair loss, hirsutism, fatigue, drowsiness, insomnia, fever, headache, premenstrual syndrome-like symptoms, altered menstrual cycle or irregular menstrual bleeding, inj-site reactions. Altered serum lipid profile.
Drug Interactions
Markedly reduced plasma conc w/ aminoglutethimide. May inhibit ciclosporin metabolism leading to increased plasma-ciclosporin conc & risk of toxicity. Antidiabetics.
