Amoxicillin trihydrate
Indications/Uses
Infections caused by susceptible gm +ve & gm -ve bacteria in respi, GI, genitourinary, skin & soft tissues infections.
Dosage/Direction for Use
Mild/moderate ear/nose/throat infection Adult 500 mg every 12 hr or 250 mg every 8 hr. Ped >3 mth 25 mg/kg day in divided doses every 12 hr or 20 mg/kg/day every 8 hr. Severe skin/skin structure & genitourinary tract infection & mild/moderate or severe lower resp infection Adult 875 mg every 12 hr or 500 mg every 8 hr. Ped >3 mth 45 mg/kg/day in divided doses every 12 hr or 40 mg/kg/day in divided doses every 8 hr. Mild/moderate or severe lower resp infection Adult 875 mg every 12 hr or 500 mg every 8 hr. Ped >3 mth 45 mg/kg/day in divided doses every 12 hr or 40 mg/kg daily in divided doses every 8 hr. H. pylori infection Adult Triple therapy: Amoxicillin 1 g + clarithromycin 500 mg + omeprazole 20 mg all given every 12 hr for 10 days. Dual therapy: Amoxicillin 1 g + lansoprazole 30 mg each given every 8 hr for 14 days. Renal impairment CrCl 10-30 mL/min Adult & childn ≥40 kg 250 or 500 mg every 12 hr, <40 kg 15 mg/kg daily in divided doses every 12 hr; CrCl <10 mL/min Adult & childn ≥40 kg 250 or 500 mg every 24 hr, <40 kg 15 mg/kg as single daily dose. Hemodialysis 250 or 500 mg every 24 hr. Neonates & infants ≤3 mth 20-30 mg/kg daily in divided doses every 12 hr.
Administration
May be taken with or without food: May be taken w/ meals for better absorption & to reduce GI discomfort.
Contraindications
Adverse Reactions
Mucocutaneous candidiasis; nausea, vomiting, diarrhea, black hairy tongue, hemorrhagic/pseudomembranous colitis; serum sickness-like reactions, erythematous maculopapular rashes, erythema multiforme, Stevens-Johnson syndrome, exfoliative dermatitis, toxic epidermal necrolysis, acute generalized exanthematous pustulosis, hypersensitivity vasculitis & urticaria; hepatic dysfunction eg, cholestatic jaundice, hepatic cholestasis & acute cytolytic hepatitis; crystalluria; hemolytic anemia, thrombocytopenia, thrombocytopenic purpura, eosinophilia, leukopenia & agranulocytosis; reversible hyperactivity, agitation, anxiety, insomnia, confusion, convulsions, behavioral changes, dizziness.
Drug Interactions
Decreased renal tubular secretion & increased & prolonged blood levels w/ probenecid. Concomitant use w/ chloramphenicol macrolides, sulfonamides & tetracyclines may interfere w/ the bactericidal effects of penicillin. May affect the gut flora, leading to lower estrogen reabsorption & reduced efficacy of combined oral estrogen/progesterone contraceptives.
