Indications/Uses
Management of renal tubular acidosis w/ Ca stones, hypocitraturic Ca oxalate nephrolithiasis of any etiology & uric acid lithiasis w/ or w/o Ca stones.
Dosage/Direction for Use
Patient w/ severe hypocitraturia (urinary citrate <150 mg/day) Initially 60 mEq/day (20 mEq tid or 15 mEq qid). Patient w/ mild to moderate hypocitraturia (>150 mg/day) Initially 30 mEq/day (10 mEq tid). Use 24 hr urinary citrate &/or urinary pH measurements to determine adequacy of initial dosage & to evaluate effectiveness of any dosage change. Measure urinary citrate &/or pH every 4 mth.
Administration
Should be taken with food.
Contraindications
Hyperkalemia or conditions predisposing to hyperkalemia eg, chronic renal failure, uncontrolled DM, acute dehydration, strenuous physical exercise in unconditioned individuals, adrenal insufficiency, extensive tissue breakdown. Concomitant use w/ K-sparing agent eg, triamterene, spironolactone or amiloride; anticholinergic medication. Patient suffering from delayed gastric emptying, esophageal compression, intestinal obstruction or structure. PUD. Renal insufficiency (GFR <0.7 mL/kg/min).
Special Precautions
Patients w/ impaired mechanisms for excreting K. Avoid in patients w/ chronic renal failure or any other conditions which impair K excretion eg, severe myocardial damage or heart failure. Discontinue immediately if severe vomiting, abdominal pain or GI bleeding occurs & investigate possibility of bowel perforation or obstruction.
Adverse Reactions
Abdominal discomfort, vomiting, diarrhea, loose bowel movements or nausea.
Drug Interactions
Severe hyperkalemia w/ K-sparing diuretic (eg, triamterene, spironolactone or amiloride).
