Levofloxacin hemihydrate
Indications/Uses
Treatment of infections caused by susceptible organisms including gm +ve bacteria.
Dosage/Direction for Use
Complicated skin infections & hospital-acquired pneumonia 750 mg once daily for 7 to 14 days. Community-acquired pneumonia, acute bacterial sinusitis 750 mg once daily for 5 days. CrCl 20-49 mL/min Initially 750 mg daily, then 750 mg every 48 hr on subsequent doses. CrCl up to 19 mL/min (including haemodialysis & peritoneal dialysis) Initially 750 mg daily, then 500 mg every 48 hr on subsequent doses.
Administration
May be taken with or without food.
Contraindications
Hypersensitivity to levofloxacin, fluoroquinolones or quinolone group.
Special Precautions
Epilepsy, history of CNS disorder; G6PD deficiency, or myasthenia gravis. Maintain adequate fluid intake to avoid the risk of crystalluria. Discontinue if if patients experience tendon pain, inflammation or rupture. Avoid exposure to strong sunlight or sunlamps. Patients w/ QT prolongation or relevant risk factors eg, uncorrected electrolyte disturbances, bradycardia, or preexisting cardiac disease. MRSA infections. Renal impairment. Pregnancy & lactation. Patients <18 yr.
Adverse Reactions
Symptomatic hyperglycaemia and/or hypoglycaemia; nausea, vomiting, diarrhoea, abdominal pain & dyspepsia; headache, dizziness, confusion, insomnia & restlessness; rash & pruritus, hypersensitivity-type reactions; anaphylaxis; arthralgia or myalgia; tendon damage; cystalluria, transient increases in serum creatinine or blood urea nitrogen; elevated liver enzyme values, jaundice & hepatitis; eosinophilia, leucopenia, thrombocytopenia; tachycardia, hypotension, edema, syncope, hot flushes & sweating.
Drug Interactions
May increase plasma conc of drugs that are metabolised by CYP450 isoenzyme eg, theophylline & tizanidine. Enhances the effect of oral anticoagulants eg, warfarin & glibenclamide. Increased plasma conc w/ probenecid. May alter serum conc of phenytoin. Potential to prolong the QT interval of class Ia antiarrhythmic drugs (eg, quinidine & procainamide) or class III antiarrhythmics (eg, amiodarone & sotalol). Caution when used w/ drugs known to prolong the QT interval (eg, astemizole & terfenadine, cisapride, erythromycin, pentamidine, phenothiazines or TCAs).
