Indications/Uses
Prevention & treatment (control) of acute & delayed nausea & vomiting associated w/ chemotherapy & RT; post-op nausea & vomiting.
Dosage/Direction for Use
Chemotherapy induced nausea & vomiting (CINV) Adult Prevention: 1-3 mg (10-40 mcg/kg) as slow IV inj (over 30 sec) or as IV infusion over 5 min, prior to the start of chemotherapy. Treatment: 1-3 mg (10-40 mcg/kg) as slow IV inj (over 30 sec) or as IV infusion over 5 min. Further doses may be administered at least 10 min apart if required. Max: 9 mg over 24 hr. Ped Prevention & treatment: 10-40 mcg/kg (up to 3 mg) as IV infusion over 5 min prior to the start of chemotherapy. May administer 1 additional dose at least 10 min after the initial infusion w/in 24-hr period if required. RT induced nausea & vomiting (RINV) Adult Prevention: 1-3 mg (10-40 mcg/kg) as slow IV inj (over 30 sec) or IV infusion over 5 min, prior to the start of RT. Post-op nausea & vomiting (PONV) Adult Prevention: 1 mg (10 mcg/kg) as slow IV inj (over 30 sec) prior to induction of anesth. Treatment: 1 mg (10 mcg/kg) as slow IV inj (over 30 sec). Max total dose: 3 mg.
Contraindications
Special Precautions
Cross-sensitivity between 5-HT3 antagonists. Closely monitor patients w/ signs of sub-acute intestinal obstruction. Patients w/ cardiac comorbidities, on cardiotoxic chemotherapy &/or w/ concomitant electrolyte abnormalities. Concomitant use w/ other serotonergic drugs. Patients w/ rare hereditary problems of galactose intolerance, lactase deficiency or glucose-galactose malabsorption. Pregnancy & lactation.
Adverse Reactions
Headache; constipation. Elevated hepatic transaminases.
Drug Interactions
Serotonin syndrome w/ other serotonergic drugs. Drugs known to prolong QT interval &/or which are arrhythmogenic.
