Sevelamer carbonate
Indications/Uses
Hyperphosphatemia in adults receiving haemodialysis or peritoneal dialysis; adults w/ CKD not on dialysis w/ serum P >1.78 mmol/L.
Dosage/Direction for Use
Initially 2.4-4.8 g daily based on clinical needs & serum P level to be taken tid. Patient previously on phosphate binders (sevelamer HCl or Ca based) Given on a g for g basis w/ monitoring of serum P levels.
Administration
Should be taken with food: Swallow whole, do not chew/crush/divide.
Contraindications
Hypersensitivity. Hypophosphatemia. Bowel obstruction.
Special Precautions
Not recommended in adults w/ CKD not on dialysis w/ serum P <1.78 mmol/L. Dysphagia swallowing disorders, severe GI motility disorder including untreated or severe gastroparesis, retention of gastric contents & abnormal or irregular bowel motion. Active inflammatory bowel disease, major GIT surgery. Intestinal obstruction & ileus/subileus. Assess vit A, D, E, & K regularly. Folate deficiency. Monitor serum Ca levels & serum bicarbonate at regular interval. Peritonitis. Patients taking anti-arrhythmias & anti-seizures. Hypothyroidism. Long-term chronic treatment (>1 yr). Not indicated for the control of hyperparathyroidism. Avoid in patients w/ galactose intolerance, the Lapp lactase deficiency or glucose-galactose malabsorption. Pregnancy & lactation. Childn <18 yr.
Adverse Reactions
Nausea, vomiting, upper abdominal pain, constipation. Diarrhoea, dyspepsia, flatulence, abdominal pain.
Drug Interactions
Decreased bioavailability of ciprofloxacin. Reduced levels of ciclosporin, mycophenolate mofetil & tacrolimus. Rare cases of hypothyroidism w/ levothyroxine. Anti-arrhythmics & anti-seizures.
