Indications/Uses
Renal tubular acidosis w/ Ca stones, hypocitraturic Ca oxalate nephrolithiasis of any etiology & uric acid lithiasis w/ or w/o Ca stones.
Dosage/Direction for Use
Severe hypocitraturia (urinary citrate <150 mg/day) 60 mEq/day (20 mEq tid or 15 mEq qid). Mild-moderate hypocitraturia (>150 mg/day) 30 mEq/day (10 mEq tid). Measure urinary citrate &/or pH every 4 mth.
Administration
Should be taken with food: Take w/ meals or w/in 30 min after meals or bedtime snack.
Contraindications
Hyperkalemia or conditions predisposing to hyperkalemia. Chronic renal failure, uncontrolled DM, acute dehydration, strenuous physical exercise in unconditioned individuals, adrenal insufficiency, extensive tissue breakdown; PUD & renal insufficiency (GFR <0.7 mL/kg/min). Patients in whom there is cause for arrest or delay in tab passage through GI tract eg, those w/ delayed gastric emptying, esophageal compression, intestinal obstruction/stricture or use of anticholinergics. K-sparing agent (eg, triamterene, spironolactone or amiloride).
Special Precautions
Hyperkalemia & cardiac arrest. Avoid use in chronic renal failure or other condition which impairs K excretion eg, severe myocardial damage or heart failure. Concomitant use w/ diuretics & digitalis prep. Tarry stools or other evidence of GI bleeding may be noticed.
Adverse Reactions
GI complaints eg, abdominal discomfort, vomiting, diarrhea, loose bowel movements or nausea.
Drug Interactions
Hyperkalemia may occur w/ K-sparing diuretics eg, triamterene, spironolactone or amiloride.
