Isotretinoin
Indications/Uses
Severe forms of acne (nodulo-cystic forms); cystic acne & acne conglobata especially when the lesions involve the trunk.
Dosage/Direction for Use
Initially 0.5-1 mg/kg once daily or 2 divided doses. May adjust dose after 4 wk up to 1 mg/kg daily according to response & side effects. Patient intolerant to the initial dose Continue treatment at 0.1-0.2 mg/kg daily. Doses up to 2 mg/kg daily are permitted whose disease is very severe or primarily on the body instead of the face. Duration: 12-20 wk or until the total cyst count decreased by 70%.
Administration
Should be taken with food.
Contraindications
Pregnancy.
Special Precautions
Patients receiving isotretinoin should not donate blood during or for 1 mth after cessation of therapy. History of depression; monitor for signs of depressive illness. Measure liver function & fasting blood lipids after the 1st mth & thereafter. Monitor blood glucose throughout treatment in patients who either have, or are predisposed to DM. May experience reduced tolerance to contact lenses. Avoid excessive exposure to sunlight & UV light. Avoid pregnancy during treatment & for 1 mth after treatment has been w/drawn.
Adverse Reactions
Dryness of the mucous membranes & of the skin w/ scaling, fragility & erythema, especially of the face, cheilitis, pruritus, epistaxis, conjunctivitis, dry sore mouth & palmoplantar exfoliation; corneal opacities, dry eyes, visual disturbances, skeletal hyperostosis & musculoskeletal symptoms; elevation of serum triglycerides, hepatic enzymes, erythrocyte sedimentation rate & blood glucose; pancreatitis (particularly in patients w/ high triglyceride conc).
Drug Interactions
Additive toxic effects w/ vit A (including dietary supplements). Development of benign intracranial HTN w/ tetracyclines.
