Indications/Uses
Induction & maintenance of general anesth.
Dosage/Direction for Use
Induction Initially 0.5%. Surgical anesth 1.5-3% 7-10 min. Maintenance 1-2.5% + oxygen & nitrous oxide mixt 70%. Induced hypotension 2.5-4%.
Contraindications
Hypersensitivity to isoflurane or to other halogenated agents. Known or suspected susceptibility to malignant hyperthermia; developed icterus &/or fever of unknown origin, hepatic impairment or eosinophilia.
Special Precautions
Narcotic premed or concurrent use of other resp depressant may accentuate resp depression. Increased CSF pressure. Risk of hepatotoxicity; formation of significant levels of carboxy-Hb in exposed patients. Patients w/ intracranial pressure; preexisting liver disease & CAD. Salivation & tracheobronchial secretions may be stimulated in childn. Duchenne muscular dystrophy. Employ prompt & vigorous treatment of hyperkalemia & arrhythmias; subsequent evaluation for latent neuromuscular disease. May affect ability to drive or operate machinery. Pregnancy & lactation.
Adverse Reactions
Hypotension, resp depression & arrhythmias. Increased heart rate.
Drug Interactions
Predisposition to arrhythmias w/ other β-sympathomimetics eg, amphetamines. Marked hypotension w/ Ca antagonists especially dihydropyridine class; unpredictable hypotension w/ other vasodilators eg, ACE inhibitors (eg, captopril, enalapril, lisinopril) or α1-adrenoceptor antagonists (eg, prazosin). Enhanced anesth effects w/ MAOIs. Risk of hepatotoxicity w/ INH. Potentiated resp depressant effects w/ opioid analgesics. β-blockers.
