Norepinephrine bitartrate
Indications/Uses
IV infusion: Emergency treatment of collapse & in the restoration & keeping of arterial BP. Local gastric irrigation: For digestive haemorrhage, in addition to the usual treatments.
Dosage/Direction for Use
Initially 0.1-0.3 mcg/kg/min. Septic shock/haemorrhagic shock 3-5 mcg/kg/min. Digestive haemorrhage 8-16 mg added to very cold physiological serum used for gastric washing.
Special Precautions
Not to be used in hypotensive patients in whom circulatory collapse is associated w/ hypovolaemia except as an emergency measure to maintain supply to the coronary & cerebral arteries until blood vol replacement therapy can be instituted. Major left ventricular dysfunction, acute coronary insufficiency, recent MI, heart rhythm disorders arriving during treatment. Continuously check arterial BP & heart rate during infusion. Must be administered separately w/ total blood or blood plasma.
Adverse Reactions
Anxiety, difficulty in breathing, headaches, tremors. Skin necrosis (in the event of extravasation); retrosternal or pharyngeal pain, photophobia, pallor, sweating, vomiting, tachycardia, bradycardia.
Drug Interactions
Severe ventricular arrhythmia w/ volatile halogen anaesth. Paroxysmal HTN w/ the possibility of arrhythmia w/ imipramine & serotoninergic-adrenergic antidepressants. Risk of increased pressor action w/ non-selective MAOI & selective MAOI-A inhibitors, & linezolid.
