Treatment of venous thromboembolism, management of unstable angina or acute peripheral arterial embolism IV loading dose of 5,000-10,000 u, then by continuous IV infusion of 1,000-2,000 u/hr or SC inj of 15,000 u 12 hrly or intermittent IV inj of 5,000-10,000 u 4-6 hrly.
Childn Lower IV loading dose then maintenance w/ continuous IV infusion of 15-25 u/kg/hr or SC inj of 250 u/kg/hr.
Prophylaxis of post-op venous thromboembolism, prevention of thromboembolism during pregnancy w/ history of DVT or pulmonary embolism 5,000 u SC 2 hr pre-op then 8-12 hrly for 7 days until the patient is ambulant. May be increased to 10,000 u 12 hrly during 3rd trimester of pregnancy.
Prevention of re-occlusion of the coronary arteries following thrombolytic therapy in MI 2,000 u of heparin IV followed by 12,500 u SC 12 hrly after streptokinase or 5,000 u IV followed by 1,000 u/hr IV after alteplase; 12,500 u SC 12 hrly for at least 10 days to prevent mural thrombosis.
