Patient not taking a phosphate binder Initially 800-1,600 mg or one to two 800-mg FC tab based on serum P level. Serum P ≥9, ≥7.5 & <9 mg/dL: Initially 2 tab tid w/ meal; >5.5 & <7.5 mg/dL: Initially 1 tab tid w/ meal.
Patient switching from sevelamer HCl Prescribe on a gram per gram basis. Titrate further to desired phosphate levels. Max: 14 g/day.
Patient switching from Ca acetate Start dose based on current Ca acetate dose. Ca acetate one 667-mg tab: one 800-mg FC tab; two 667-mg tab: two 800-mg FC tab; three 667-mg tab: three 800-mg FC tab. Dose titration for all patients may be increased/decreased by 1 tab/meal at 2 wk intervals to reach serum P w/in 3.5-5.5 mg/dL target range.
