Indications/Uses
Control of serum P in patients w/ chronic kidney disease on dialysis.
Dosage/Direction for Use
Patient not taking a phosphate binder Initially 800-1,600 mg or one to two 800-mg FC tab based on serum P level. Serum P ≥9, ≥7.5 & <9 mg/dL: Initially 2 tab tid w/ meal; >5.5 & <7.5 mg/dL: Initially 1 tab tid w/ meal. Patient switching from sevelamer HCl Prescribe on a gram per gram basis. Titrate further to desired phosphate levels. Max: 14 g/day. Patient switching from Ca acetate Start dose based on current Ca acetate dose. Ca acetate one 667-mg tab: one 800-mg FC tab; two 667-mg tab: two 800-mg FC tab; three 667-mg tab: three 800-mg FC tab. Dose titration for all patients may be increased/decreased by 1 tab/meal at 2 wk intervals to reach serum P w/in 3.5-5.5 mg/dL target range.
Administration
Should be taken with food.
Contraindications
Hypophosphatemia or bowel obstruction.
Special Precautions
Dysphagia, swallowing disorders, severe GI motility disorders including severe constipation or major GIT surgery. Monitor serum chemistries (bicarbonate & Cl levels); for reduced vit D, E, K (clotting factors) & folic acid levels. Closely monitor TSH levels in patients receiving levothyroxine. Patients taking anti-arrhythmic or anti-seizure medications.
Adverse Reactions
Vomiting, nausea, diarrhea, dyspepsia, abdominal pain, flatulence & constipation. Peritonitis. Pruritus, rash; abdominal pain, fecal impaction; ileus & intestinal obstruction, intestinal perforation.
Drug Interactions
Decreased bioavailability of ciprofloxacin.
