Indications/Uses
Treatment of single or multiple infections caused by susceptible organisms. Used as 1st choice drug before the results of sensitivity tests are available; in combination w/ an aminoglycoside or most other β-lactams; w/ an antibiotic against anaerobes when the presence of Bacteroides fragilis is suspected. Infections including severe infections eg, septicemia, bacteremia, peritonitis, meningitis & infections in immunosuppressed & ICU patients; resp tract infection including lung infections in cystic fibrosis; ENT; UTI; skin, soft tissue; GIT, biliary, abdominal; bone & joint. Infection associated w/ hemo- & peritoneal dialysis & w/ continuous ambulatory peritoneal dialysis (CAPD). Prophylaxis to prostatic surgery (transurethral resection).
Dosage/Direction for Use
IV/IM Adult 1-6 g/day in 2 or 3 divided doses. UTI & less serious infections 500 mg or 1 g 12 hrly. Most infections 1 g 8 hrly or 2 g 12 hrly. Very severe infections particularly in immunocompromised patients 2 g 8 or 12 hrly or 3 g 12 hrly. Fibrocystic w/ pseudomonal lung infections 100-150 mg/kg/day in 3 divided doses. Prophylaxis in prostatic surgery 1 g at the induction of anesth. Inj 2nd dose during catheter removal. Infant & childn >2 mth 30-100 mg/kg/day in 2 or 3 divided doses. Immunocompromised or fibrocystic childn or w/ meningitis Up to 150 mg/kg/day in 3 divided dose. Max: 6 g/day. Neonate (0-2 mth) 25-60 mg/kg/day in 2 divided doses. Elderly (>80 yr) Max: 3 g/day.
Contraindications
Hypersensitivity cephalosporins, ceftazidime pentahydrate.
Special Precautions
History of hypersensitivity reactions to ceftazidime, cephalosporins, penicillins or other drugs. Discontinue if allergic reaction or prolonged/significant diarrhea occurs. Concurrent treatment w/ high doses of cephalosporins & nephrotoxic drugs eg, aminoglycosides or potent diuretics (eg, furosemide) may adversely affect renal function. Prolonged use may result in the overgrowth of non-susceptible organisms. Discontinue if prolonged or significant diarrhea occurs (pseudomembranous colitis). Patients under Na restriction (containing 52-mg Na). Renal impairment. Consider periodic susceptibility testing. Pregnancy & lactation.
Adverse Reactions
Eosinophilia, thrombocytosis; phlebitis or thrombophlebitis (IV administration); diarrhea; transient elevations in ALT (SGPT), AST (SGOT), LDH, GG & alkaline phosphatase; maculopapular or urticarial rash; pain &/or inflammation after IM inj; +ve Coombs' test.
Drug Interactions
May adversely affect renal function w/ nephrotoxic drugs (high-doses). Possibility of antagonism w/ chloramphenicol. Lower estrogen reabsorption & reduced efficacy of combined OCs. Slight interference may occur w/ Cu reduction methods (Benedict's, Fehling's, Clinitest).
