Indications/Uses
Infections of the resp tract, skin & skin structure, biliary tract, bone & joint, genital (ie, prostatitis, epididymis); septicemia; endocarditis. Perioperative prophylaxis.
Dosage/Direction for Use
IV/IM Adult Moderate to severe infections 500 mg-1 g every 6-8 hr. Mild infection caused by susceptible gm +ve cocci 250-500 mg every 8 hr. Acute, uncomplicated UTI 1 g every 12 hr. Pneumococcal pneumonia 500 mg every 12 hr. Severe life-threatening infections (eg, endocarditis, septicemia) 1-1.5 g every 6 hr. Perioperative prophylaxis 1 g ½-1 hr prior to procedure. Lengthy operative procedures (≥2 hr) 500 mg-1 g during operation, administration modified depending on the duration of the operative procedure. Post-op 500 mg-1 g every 6-8 hr for 24 hr.
Contraindications
Special Precautions
Hypersensitivity to cefazolin, cephalosporins, penicillins, or other drugs. Mild to life threatening pseudomembranous colitis. Prolonged use may result in overgrowth of nonsusceptible organisms. Impaired renal function. Seizures. History of GI disease, particularly colitis. May cause fall in prothrombin activity, those at risk include patients w/ renal or hepatic impairment or poor nutritional state, patients receiving a protracted course of antimicrobial therapy, & previously stabilized on anticoagulant therapy. Premature infants & neonates. Pregnancy & lactation.
Adverse Reactions
Diarrhea, oral candidiasis (oral thrush), stomach cramps, anorexia & pseudomembranous colitis; anaphylaxis, eosinophilia, itching, drug fever, skin rash, SJS; neutropenia, leukopenia, thrombocytopenia, thrombocythemia; transient rise in SGOT, SGPT & alkaline phosphatase levels; increased BUN & creatinine levels, renal failure; genital & anal pruritus (including vulvar pruritus, genital moniliasis & vaginitis).
Drug Interactions
May decreased renal tubular section w/ probenecid. False +ve reaction for glucose in the urine may occur w/ Benedict's soln, Fehling's soln or w/ Clinitest tab. +ve direct & indirect antiglobulin (Coombs) tests.
